The patient is an 18-year-old male with pain in the right lower quadrant of the abdomen and anorexia over the previous 48h, with guarding and rebound tenderness in the right iliac fossa. Blood work demonstrated a leukocytosis of 14000×109/L and CRP of 0.2mg/dL. The ultrasound showed a subhepatic appendix in contact with a mass measuring 2×2cm coming from the liver. Laparoscopy revealed evidence of a hyperaemic appendix in contact with a pedunculated mass measuring 5×4cm, which was necrotic, originating in liver segment V and compatible with torsion of Riedel's lobe of the liver (Fig. 1). Hepatectomy of the lobe and appendectomy were performed.
