A 79-year-old man with a history of smoking, atrial fibrillation and hypertension had undergone a femoral-popliteal bypass in the third portion using an in situ internal saphenous vein graft in the left lower limb due to pain at rest.

Ten years later, he came to the emergency room due to a pulsating mass in the left thigh. Echo-Doppler demonstrated a true saccular aneurysm of the vein bypass that was 3.7cm in diameter. This finding was confirmed by computed tomography angiography (Fig. 1).

The surgical repair involved resection of the aneurysmal segment, which was replaced by an endarterectomized superficial femoral artery patch (Fig. 2).
