The patient was 65-year-old female with large cell lung carcinoma T2N0M1 treated with chemotherapy and surgery 4 years before. A year ago, she presented a single right adrenal metastasis with infiltration of the inferior vena cava, the right superior polar artery, the right renal vein –thrombosed- and the diaphragmatic pillar (Fig. 1). She was treated with chemoradiotherapy and a good initial response was obtained with subsequent tumour growth. Given the impossibility of continuing irradiation, the decision was made to perform surgery. Right adrenalectomy and nephrectomy were carried out with the replacement of the infrahepatic vena cava by a cryopreserved graft (Fig. 2); the left renal vein (arrow) was preserved.
