The patient is a 36-year-old male who came to the emergency room with abdominal pain that had been progressing over the previous 24h, and abdominal distension. Abdominopelvic computed tomography scan detected a right paracolic collection (Fig. 1, axial projection) in the periumbilical area, with a hyperdense collection of calcium density and air bubbles inside. Exploratory laparotomy revealed a perforated Meckel's diverticulum of the ileum, at whose distal end a saccular structure was observed with lithiasic material in its interior (Fig. 2). We performed simple diverticulectomy of the perforated diverticulum using an endocutter. The patient progressed favorably, except for a surgical would infection, and he was discharged from the hospital 12 days later.
