We present the case of a 49-year-old woman who was referred to our department for abdominal pain and toxic syndrome. A CT scan revealed a transverse colon neoplasm associated with a right anterior diaphragmatic defect (Morgagni hernia) containing the entire colon proximal to the neoplasm. The patient was scheduled for extended right hemicolectomy and laparoscopy-assisted hernia repair.
The first step was to reduce the content of the diaphragmatic hernia, followed by the resection of the hernia sac. The defect had a diameter of 5cm and was repaired using Proceed® mesh (15×15cm) affixed with transcutaneous polypropylene sutures and with metal tackers. Afterwards, an extended right hemicolectomy was performed through a mini upper abdominal midline laparotomy. The patient presented a postoperative period without incident and was discharged on the 7th postoperative day (Video).
