A 70-year-old male with a history of hydatidosis of the duodenum that had been treated surgically 30 years before came to our consultation due to a painful mass at the laparotomy scar. While cleaning the area, the wound opened spontaneously and oozed serous material and hydatids. Abdominal ultrasound and CT scan revealed a collection with hypodense content in liver segment V that had fistulized to the skin (Figs. 1 and 2). The wound was explored surgically, and a multiperforated drain was inserted (Fig. 3, arrows: hydatids). The patient received albendazole and lavages with hypertonic saline for 2 weeks until clinical-radiological resolution of the condition. The microbiology study was negative and the pathology study confirmed the diagnosis of hydatidosis.
