A 74-year-old mala patient was being studied for weight loss and a mass in the right hemithorax, which appeared after a coughing episode one year before and had progressively increased in size.

Colonoscopy and thoracoabdominal scan detected a sigmoid neoplasm and a double hernia defect at the diaphragmatic level and in the right thoracoabdominal lateral wall. The hernia contained the ascending and transverse colon and ascended towards the thoracic cavity, causing complete atelectasis of the right lower lobe, compatible with a transdiaphragmatic intercostal hernia (Fig. 1). Both conditions were treated in a single laparoscopic procedure, repairing the hernia with a double prosthesis.
