The incidence of syphilis is increasing. Syphilitic uveitis may present clinically as an outer retinopathy with different patterns.1 On optical coherence tomography (OCT), its appearance resembles acute zonal outer retinopathy. The ellipsoid zone line disappears or is interrupted, and the external limiting membrane (ELM) line is attenuated. Some authors have referred to this as ellipsoiditis. Early diagnosis and treatment are essential. Recovery of the fine retinal structure and its function is very rapid. The OCT images correspond to presentation and to 1 and 4 weeks after initiation of penicillin therapy (Fig. 1).
