Case reports

Two asymptomatic women treated with hydroxychloroquine 200mg every day for 8 and 16 years developed retinal toxicity.

Discussion

Patient 1 was found to have a normal fundus and autofluorescence examination. Patient 2 was found to have a completely normal fundus examination. Fluorescein angiography shows parafoveal hyperfluorescence, and autofluorescence shows a minimal decrease in signal in the same region. In both patients the SD-OCT shows disruption of the ellipsoid zone in parafoveal region (“flying saucer” sign). SD-OCT findings in the retina can identify hydroxychloroquine retinopathy in asymptomatic patients.

Casos clínicos

Dos pacientes asintomáticas, tratadas con hidroxicloroquina 200mg una vez al día durante 8 y 16 años, desarrollaron toxicidad retiniana.

Discusión

En la paciente 1 el fondo de ojo y la autofluorescencia eran normales. La paciente 2 tenía un fondo de ojo normal, la angiografía fluoresceínica mostró hiperfluorescencia parafoveal y la autofluorescencia presentó una leve disminución de la señal en la misma región. En las 2 pacientes la SD-OCT mostró pérdida de la banda elipsoide en la región parafoveal («signo del platillo volante»). La SD-OCT puede identificar toxicidad retiniana antes que el paciente presente pérdida de visión.
