A 54-year-old man presented to the ophthalmic emergency department of our center with eye pain and blurred vision in his right eye following a workplace accident. Examination revealed a penetrating corneal injury with the presence of an intraocular foreign body (IOFB) involving the corneoscleral limbus, perforating the cornea, iris, anterior lens capsule, and lens. Immediate surgical intervention was carried out with the extraction of the IOFB, identified as an 8mm mussel shell fragment, and the removal of the resulting traumatic cataract. Both preoperative and postoperative examinations showed an attached retina with no signs of retinal tears or vitreous hemorrhage. Appropriate management in this case, along with the timely identification of the agent, led to favorable outcomes despite the size of the intraocular foreign body.
Un hombre de 54 años acudió al departamento de urgencia oftalmológica de nuestro centro con dolor ocular y visión borrosa en su ojo derecho posterior a un accidente laboral. El examen reveló una lesión penetrante corneal con la presencia de un cuerpo extraño intraocular (CEIO) involucrando el limbo corneo-escleral, perforando la córnea, el iris, la cápsula anterior del cristalino y el cristalino. Se procedió a la intervención quirúrgica inmediata con la extracción del CEIO, el cual se logró identificar como un trozo de concha de mejillón con un tamaño de 8mm y la extracción de la catarata traumática producida. Durante los exámenes preoperatorios como postoperatorios, se observó una retina adherida sin signos de desgarros retinianos o hemorragia vítrea. Aunque las lesiones abiertas en el globo ocular con cuerpo extraño intraocular (CEIO) representan un desafío, la identificación rápida y el manejo adecuado en este caso condujeron a resultados favorables.
