A 49-year-old woman attended the emergency department with neck pain disturbing her sleep at night, functional impotence, particularly on neck extension movements and dysphagia. She displayed no fever or accompanying signs. Physical examination revealed swelling of the posterior pharyngeal wall in the rhino- to oropharyngeal tract, which was hard on palpation. Fibroscopy showed asymmetry of the posterior wall.

An emergency computed tomography scan was performed (Fig. 1, CT), showing an increase in size of the retropharyngeal soft tissues caused by a fluid collection (asterisk) and thick prevertebral calcifications (black arrow) at the height of CT. After 48h, a cervical MRI scan was performed (Fig. 2) showing that the retropharyngeal fluid collection had reduced in size (white arrow).


Given the symptoms and the findings on the imaging tests (particularly the CT), this picture was diagnosed as tendinitis of the longus colli muscle. The prescribed treatment was analgesia and the patient was discharged 72h after her admission due to clinical improvement.

The differential diagnosis of this disorder includes diseases as severe as retropharyngeal abscess, due to the fluid collection, or vertebral fractures, due to the calcifications. An absence of adenopathies or previous trauma is helpful, respectively. Although this is a rare disease it is important to be aware of it to avoid unnecessary diagnostic or therapeutic procedures.
