La presencia de hernias meningoencefálicas en el oído medio ha tenido un súbito descenso con el advenimiento de la microcirugía, antibioterapia y nuevas técnicas quirúrgicas empleadas en el tratamiento de patologías crónicas del oído, volviéndose infrecuentes en la actualidad. Nuestro objetivo es describir 4 casos, tres postquirúrgicos y uno postinflamatorio, tratados en nuestro servicio, haciendo hincapié en el diagnóstico, las técnicas quirúrgicas utilizadas para la reducción y las complicaciones que hemos tenido. En nuestra experiencia las hernias meningoencefálicas pueden ser asintomáticas en su gran mayoría y son diagnosticadas casualmente. Necesitan corrección inmediata por su potencial riesgo y la vía transmastoidea es la más utilizada, aunque en algunas ocasiones se realiza un abordaje transtemporal por fosa media
Meningoencephalic hernia of the middle ear, due to the advent of antibiotics, the surgical microscope, and the new surgical techniques for the treatment of chronic otitis media, is an unfrequent pathology in the presents days. The goal of this paper is to present four cases of meningoencephalic hernia of the middle ear, three of then postsurgical and the remaining of inflammatory origin. Diagnosis, treatment and complications are discussed. Meningoencephalic hernia of the middle ear is usually asymptomatic and diagnosis is accidental. Immediate surgical treatment is indicated in order to avoid the potential complications of this disease.
