Therefore, in order to carry out metabolic studies, parameters obtained by relating the concentration of two or more substances in plasma and in untimed urine samples are used.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Urinary ratios (UR) compose the simplest way to express the excretion of solutes. It is based on the fact that creatinine clearance, in the absence of kidney failure, is constant. In micturitions without measurement of volume per unit of time, knowing the plasma and urinary concentrations of creatinine and of the substances we want to study, we can calculate the clearance ratio or fractional excretion (FE) of sodium (Na), potassium (K) and chlorine (Cl), as well as excretion ratios (ER) of calcium (Ca), uric acid, magnesium (Mg), citrate and oxalate. Phosphate (P) is usually expressed as phosphate reabsorption rate (PRR).<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The objectives of this work were:</p><p id="par0025" class="elsevierStylePara elsevierViewall">To correlate, within the same urine sample, the urinary excretion of substances obtained per unit of time, with data that are calculated by measuring the concentration of these substances in blood and urine. The latter parameters do not depend on the amount of urine produced per unit of time.</p><p id="par0030" class="elsevierStylePara elsevierViewall">On the one hand, to analyze the association of the values of the milliequivalents of Na, K and Cl that are excreted in 24 h (mEq/kg/day), with formulas that relate the concentration of these substances in blood and urine (UR and FE).</p><p id="par0035" class="elsevierStylePara elsevierViewall">On the other hand, to examine the relationship of the amount of Ca, Mg, uric acid, P, citrate and oxalate eliminated in urine in 24 h (mg/kg/day), with parameters that relate the concentration of these substances in blood and urine (UR and ER).</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Material and methods</span><p id="par0040" class="elsevierStylePara elsevierViewall">It is a cross sectional, analytical study that included children of three and 14 years old. The study period was 2000−2012. The evaluated children came from the outpatient clinic of several pediatric specialties. Healthy siblings of children who were being monitored in the pediatric nephrology outpatient clinic were also included in the study. The biochemical study was carried out on an outpatient basis and under the child’s usual living and feeding habits. Four hundred one healthy children (187 boys and 214 girls) were evaluated, with a mean age of 8.78 ± 3.40, who met the following criteria of normality: absence of metabolic, neuromuscular, endocrine and renal disease. Those with a personal history of renal lithiasis, abnormalities in the urine test strip and on medication treatment, were excluded. Age, sex, weight, height were recorded and the body surface area (BSA) was calculated. The following parameters were determined in blood and 24-h urine: creatinine (Cr), Na, K, Cl, Ca, Mg, uric acid and P. Urinary oxalate and citrate were also determined in 114 children (58 boys and 56 girls).</p><p id="par0045" class="elsevierStylePara elsevierViewall">Creatinine clearance (CCr) should be greater than 80 mL/min/1.73 m². It was estimated that the collection was correct when creatinuria ranged between 15 and 25 mg/kg/day.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> In all patients, creatinuria was calculated and expressed as mg/kg/24 h; urine Na amount (mEq/kg/24 h); K (mEq/kg/24 h), Cl (mEq/kg/24 h), Ca (mg/kg/24 h), Mg (mg/kg/24 h), P (mg/kg/24 h), uric acid (mg/kg/24 h), citrate (mg/kg/24 h) and oxalate (mg/kg/24 h).</p><p id="par0050" class="elsevierStylePara elsevierViewall">The following were calculated for all patients:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">none-</span><p id="par0055" class="elsevierStylePara elsevierViewall">Body surface area (BSA) m<span class="elsevierStyleSup">2</span>: √weight (KG) height (cm)3600</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">none-</span><p id="par0060" class="elsevierStylePara elsevierViewall">Creatinine clearance (CCr) in 24-h urine, expressed as mL/minute/1.73 m² BSA, the following formula was used:</p></li></ul></p><p id="par0065" class="elsevierStylePara elsevierViewall">(UCr × volume of urine in one minute/PCr) × (1.73/m² BSA).</p><p id="par0070" class="elsevierStylePara elsevierViewall">Where: PCr is plasma creatinine (mg/dL); UCr is urinary creatinine (mg/dL).<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">none-</span><p id="par0075" class="elsevierStylePara elsevierViewall">Urinary Ratio (UR): expresses the mg or mEq of substance in urine (US) for each mg of filtered Cr, always taking into account that the volume unit is the same:</p></li></ul></p><p id="par0080" class="elsevierStylePara elsevierViewall">US (mg or mEq/dL)/ urine UCr (mg/dL).<ul class="elsevierStyleList" id="lis0015"><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">none-</span><p id="par0085" class="elsevierStylePara elsevierViewall">Fractional excretion (FE) of a substance is defined as the volume of blood (ml) that is free of that substance (S) per 100 mL glomerular filtration (GF):</p></li></ul></p><p id="par0090" class="elsevierStylePara elsevierViewall">FE = [(PCr × US) × 100] ÷ (UCr × PS).<ul class="elsevierStyleList" id="lis0020"><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">none-</span><p id="par0095" class="elsevierStylePara elsevierViewall">Tubular reabsorption of phosphate per 100 mL GF</p></li></ul></p><p id="par0100" class="elsevierStylePara elsevierViewall">TRP = 100- ([(PCr × UP) × 100] ÷ (UCr x PP)).<ul class="elsevierStyleList" id="lis0025"><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">none-</span><p id="par0105" class="elsevierStylePara elsevierViewall">Excretion rate of a substance (ER): the excretion of a substance (mg or mEq) corresponding to 100 mL of GF:</p></li></ul></p><p id="par0110" class="elsevierStylePara elsevierViewall">ER = (PCr × US) ÷ (UCr).</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0115" class="elsevierStylePara elsevierViewall">It was carried out with the SPSS® program. The data were presented as mean with standard deviation M ± SD. The Student's <span class="elsevierStyleItalic">t</span>-test was used to compare the continuous variables. The Pearson correlation coefficient was used to study the relationship between two variables. A <span class="elsevierStyleItalic">P</span> < .05 value was considered significant.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The children's parents or guardians gave their consent for their inclusion in the study.</p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0125" class="elsevierStylePara elsevierViewall">The mean and standard deviation values of the parameters studied are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. No differences were found in terms of gender when comparing the mean values of the parameters.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">Children with a BMI greater than 22 (n = 52) were compared with others of similar age and BMI lower than 22 (n = 53). We found no significant differences in the mean values of the determinations.</p><p id="par0135" class="elsevierStylePara elsevierViewall">As we can see in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, there was a very good correlation between the quantified excretion of Na values (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>), K and Cl (mEq/kg/day) with the UR. Correlations between the measurements of these solutes in 24-h urine tests and the FEs were also positive, although with a lower correlation coefficient (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">In the same way, the urinary ratios of Ca (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>), Mg (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>), P (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>), uric acid (<a class="elsevierStyleCrossRef" href="#fig0025">Fig. 5</a>), citrate (<a class="elsevierStyleCrossRef" href="#fig0030">Fig. 6</a>) and oxalate maintained an excellent correlation with the urinary excretion of these substances in 24-h urine tests (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). The relationship between 24-h urine determinations and ERs, although to a lesser extent, remained positive.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><elsevierMultimedia ident="fig0020"></elsevierMultimedia><elsevierMultimedia ident="fig0025"></elsevierMultimedia><elsevierMultimedia ident="fig0030"></elsevierMultimedia><p id="par0145" class="elsevierStylePara elsevierViewall">Most URs varied significantly with age (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). In contrast, the significance of the correlations of most FE and ER with age had little biological implication given the low value of r.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0150" class="elsevierStylePara elsevierViewall">In metabolic studies performed for the diagnosis or follow-up of patients with renal lithiasis, 24-h urine has been used for a long time to analyze the urinary excretion of the different factors involved in the process of lithiasis. However, obtaining 24-h urine in young or incontinent children is extremely difficult. Therefore, several equations have emerged in an attempt to obtain an estimate of solutes excretion in urine, based on the concentration of Cr in urine and/or serum.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">UR is the most basic functional test that does not require simultaneous blood draw. In the same way as UR, there are other somewhat more complex functional parameters, FEs and ERs, which also avoid the errors that may exist with timed urine collection.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3,5</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">The normal values of the most commonly used URs, FEs and ERs on pediatric age are well known.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,6–10</span></a> Coinciding with the work of these authors, UR values showed variations with growth.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,6–10</span></a> On the contrary, FE and ER correlations with age, although significant, had little biological implication. No differences were found in terms of gender when comparing the mean values of the parameters. Nor did the parameters of children with BMI > 22 differ from those of children with BMI < 22. In our population, the values of phosphaturia and citraturia were higher than those of other authors, probably due to environmental or genetic factors.</p><p id="par0165" class="elsevierStylePara elsevierViewall">In our cohort, the values of the metabolites determined in 24-h urine involved in lithogenesis and the UR values showed an excellent correlation, indicating that high amounts of these solutes in 24-h urine correspond to high UR values of these substances. Similarly, the FE and ER figures also showed good correlation with the values collected in timed urine.</p><p id="par0170" class="elsevierStylePara elsevierViewall">Hypercalciuria has been repeatedly described in the literature as the most common lithogenic risk factor.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11–14</span></a> The results of our work confirm a well-known fact, that calcium determined in 24-h urine has a good correlation with the values of Ca/Cr ratio and CaER.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,10</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">There are other physicochemical factors in urine that play an important role in the complex process that originates renal lithiasis, either by promoting (uric acid, P, Na, oxalate) or inhibiting (citrate, Mg) the development of lithiasis.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,15</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">Regarding uric acid, high values of uric acid/Cr ratio or uric acid ER in the presence of normal uric acid levels, would indicate an excessive intake of animal protein in the diet. Similarly, if the P/Cr ratio is elevated in normal phosphocalcic metabolism analysis, an excess of protein in the diet should be considered.</p><p id="par0185" class="elsevierStylePara elsevierViewall">Under normal conditions, Na excretion is equal to Na intake.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> Therefore, if kidney function is not impaired, a high Na/Cr ratio or NaFE indicate high dietary salt intake. Similarly, the K/Cr or KFE ratios are valid for estimating renal potassium excretion.</p><p id="par0190" class="elsevierStylePara elsevierViewall">Low molecular weight inhibitors (citrate and magnesium) have been shown to be higher in pediatric age and in children with hypercalciuria without associated lithiasis.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14,17</span></a></p><p id="par0195" class="elsevierStylePara elsevierViewall">However, it should be noted that the values for the same equation may vary, depending on the time of day when the sample is collected. Several studies show increased lithogenic risk in the composition of urine during the night period, which is manifested by an increase in Ca excretion and an increase in the Ca/citrate ratio in the first urine of the day.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,19</span></a> The Ca/Cr ratio is also higher one hour after dairy intake. For this reason, some authors advise the determination of Ca, citrate and Cr in isolated urine samples at two different times of the day, before dinner and in the first morning urine. If the Ca/citrate ratio in any urine sample is greater than 0.33 mg/mg, there is a risk of urinary crystallization.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0200" class="elsevierStylePara elsevierViewall">In addition, it should be kept in mind that FEs rise as GF decreases, so the interpretation of a given value always requires knowing the GF value.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><p id="par0205" class="elsevierStylePara elsevierViewall">In summary, there is good correlation between the values of calcium, sodium, uric acid, phosphate, oxalate, citrate, magnesium and potassium determined in 24-h urine and urinary ratios, fractional excretions and excretion ratios of these solutes. In these parameters, the amount of urine produced does not intervene but the concentration of these substances in urine and blood does, without having to resort to timed urine which presents great difficulties among the pediatric population. In daily practice, these equations provide very useful information regarding the study of the etiology, especially of the nutritional factors, that condition renal lithiasis in children, as well as in terms of the adherence to dietary treatment.
