Multiple sclerosis (MS) is an inflammatory, demyelinating, and neurodegenerative disease that affects the central nervous system. Adherence to treatment is essential to modify the course of the disease. Cognitive impairment affects up to 70% of these patients and is a limitation for administration and adherence to treatment.

The main objective was to assess the degree of adherence to treatment in patients with MS and cognitive impairment. Secondary objectives were to describe the tools that can influence adherence to treatment and to evaluate the effectiveness of the technological devices used to alert the patient that he/she must self-inject.

The method consisted of a descriptive, retrospective, and cross-sectional pilot study. It was carried out in the MS clinic of the Torrecárdenas University Hospital from December 2018 to September 2019. The sample included 20 patients diagnosed with MS currently treated with immunomodulatory injectable drugs for at least 6 months. The Symbol Digit Modalities Test (SDMT) was used to assess cognitive impairment and to determine adherence, patients answered the Morisky-Green test (MG).

Twenty patients (8 men and 12 women) were included over 10 months with MS and cognitive impairment (SDMT score less than 50th percentile). Regarding gender-differentiated adherence, 75% of men were not adherent to treatment; by contrast, 66.6% of women were com-pliant with the dosage schedule. There were no statistically significant differences in terms of differentiated adherence by type of treatment. The main methods used to promote adherence were: 10% calendar, 20% family members, and 30% alarms as reminder.

Adherence results show a predominance of adherence in women, even with cognitive impairment. This reinforces our initial hypothesis that relates good therapeutic adherence to lesser progression of cognitive impairment in MS. As a preventive measure, we would like to emphasize the importance of the role of the nursing team in detecting cognitive impairment and in assessing adherence to treatment.

La esclerosis múltiple (EM) es una enfermedad inflamatoria, desmielinizante y neurodegenerativa que afecta al sistema nervioso central. La adherencia al tratamiento resulta imprescindible para modificar el curso de la enfermedad. El trastorno cognitivo afecta hasta el 70% de estos pacientes y supone una limitación para la administración y la adherencia al tratamiento.

El objetivo principal fue valorar el grado de adherencia al tratamiento en los pacientes con EM y trastorno cognitivo. Como objetivos secundarios, describir los recursos que pueden influir en la adherencia al tratamiento y valorar la efectividad de los dispositivos tecnológicos utilizados para avisar al paciente de que se tiene que autoinyectar.

Estudio piloto descriptivo, retrospectivo y transversal. Se llevó a cabo en la consulta de EM del Hospital universitario Torrecárdenas (Almería) durante los meses de diciembre de 2018 septiembre de 2019. La muestra incluyó 20 pacientes diagnosticados de EM tratados actualmente con fármacos inyectables inmunomoduladores que llevan al menos 6 meses. Se utilizó el test Screnning digital symbol (SDMT) para la valoración del trastorno cognitivo y para determinar la adherencia, los pacientes respondieron al test Morisky Green(MG).

Se incluyeron, durante un periodo de 10 meses, 20 pacientes (8 hombres y 12 mujeres) con EM y trastorno cognitivo (puntuación en el SDMT menor al percentil 50). Con respecto a la adherencia diferenciada entre sexos, el 75% de los hombres no fueron adherentes al tratamiento, por el contrario, el 66.6% de las mujeres presentaron cumplimiento a la pauta posológica. Los no adherentes tienen un percentil inferior a 50. No existieron diferencias estadísticamente significativas en cuanto a la adherencia diferenciada por los tipos de tratamiento. Los principales métodos para favorecer la adherencia fueron: 10% calendario, 20% un familiar y el 30% alarmas como recordatorio.

Los resultados de adherencia muestran un predominio de la adherencia en mujeres, aun teniendo trastorno cognitivo, Esto afianza nuestra hipótesis inicial que relaciona la buena adherencia terapéutica con menor progresión del trastorno cognitivo en la EM. Como medida de prevención destacamos el papel relevante del equipo de enfermería en la detección del trastorno cognitivo y en valoración la de adherencia.
