Screening by nurses and junior doctors to detect malnutrition when patients are first assessed in hospital

Abstract

Four hundred and fifty four ward nurses and 319 junior doctors in 70 hospitalswere interviewed about the last patient they had admitted, using a brief questionnaire. The availability of weighing scales and height measuring equipment was assessed by observers who visited the wards of 107 hospitals. Two thirds of nurses and doctors asked patients about recent food intake; half of the nurses and three quarters of the doctors asked about unintentional weight loss. Answers to the questions were recorded in the notes on 52–80 per cent of occasions. Two thirds of nurses weighed the patient, but only 11 per cent (%) measured height; approximately 80% of results were recorded. Most nurses and doctors who asked no questions about nutrition and made no measurements failed to do so because they regarded them as unimportant. Weighing scales were adequately provided, but height measuring equipment was available in only 17% of wards.

    Unfortunately, the intervention failed to achieve both objectives. Our findings are in line with those of several other studies, showing that nutritional knowledge and perception are low among residents; thus, malnutrition is not identified and consequently not treated [30–34]. Basic teaching in medical schools (knowledge) and training of abilities and skills in clinical nutrition is largely lacking in internships [26].

    These studies indicated that patients are more likely to be weighed in the case of planned compared to emergency surgery [3,10], as are those patients with multiple comorbidities [10]. Other factors described in the literature that predict the likelihood of weighing include inadequate and poor calibration of equipment [2]; competing priorities for nurses’ time and confusion about where to record patient weight [10] and concerns about manual handling of patients [6]. Factors that have not yet been examined include patients from culturally and linguistically diverse (CALD) background where language may present a barrier, and those in isolation due to infection control.

    The following data were retrieved from the medical records of our patients at study entry of hospital admission: age, BMI (kg/m2), percentage of weight loss before hospitalization, albumin (g/L), prealbumin (mg/L), and C-reactive protein (CRP) (mg/L), and MNA for patients older than 70 to determine the nutritional status of the patients in our study [5].
