Resumen

El presente artículo es de divulgación de un importante capítulo de la epidemiología clínica que se refiere a la evaluación de las Pruebas Diagnósticas. Recurriendo lo mínimo posible a las fórmulas matemáticas, se explica el significado llano y la interpretación funcional de los índices diagnósticos más importantes que caracterizan a las mismas, para desde este punto describir su traslado a la práctica clínica diaria, especialmente en lo que afecta a la confección de un protocolo diagnóstico. No se analizan aspectos relacionados con la medicina basada en la evidencia, lo que constituiría el siguiente escalón una vez dominados estos conceptos.

The present article aims to disseminate knowledge of a topic that is important in clinical epidemiology: evaluation of diagnostic tests. Mathematical formulae are kept to a minimum. The significance and interpretation of the most important diagnostic indexes characterizing these tests are explained in order to describe their use in daily clinical practice, especially their role in the design of diagnostic protocols. Features related to evidence-based medicine, which represent the next step after mastery of these concepts, are not analyzed.

La exactitud del FAST disminuye notablemente en los pacientes politraumáticos con fractura pélvica.
El objetivo es analizar las consecuencias de tomar decisiones terapéuticas basadas en el resultado del FAST en los pacientes politraumáticos con fractura de pelvis.
Estudio descriptivo de pacientes con politraumatismos mayores de 16 años que han ingresado en el área de críticos o que han fallecido previamente, con fractura pélvica. El resultado del FAST ha sido comparado con un valor realmente positivo o negativo según el resultado de la laparotomía o de la tomografía computarizada.
En 13 años, se ha incluido a 263 pacientes politraumáticos con fractura pélvica (ISS medio de 31; mortalidad 19%). El FAST tenía una sensibilidad del 65,2%, una especificidad del 69%, una tasa de falsos negativos del 34,8% y una tasa de falsos positivos del 30,9%. Los pacientes hemodinámicamente inestables tenían el doble de mortalidad que los pacientes estables (27% vs. 14%, p < 0,05). Los pacientes con un FAST positivo tenían mayor mortalidad que los pacientes con FAST negativo (43% vs. 26%); 4 de 10 pacientes hemodinámicamente inestables con un FAST falsamente positivo que se sometieron a laparotomía exploradora innecesaria murieron por shock hipovolémico. La mortalidad se redujo del 60 al 20% asociando un packing preperitoneal.
La reducida eficacia del FAST en pacientes con fractura de pelvis nos obliga a cuestionarnos las consecuencias de la toma de decisiones terapéuticas con base en sus resultados. Los pacientes con FAST falsamente positivo tienen una mortalidad mayor, que se puede reducir aplicando un packing preperitoneal.
FAST is essential to decide if trauma patients need laparotomy, but has a notably decrease in accuracy in patients with pelvic fracture.
Our objective is to analyze the consequences of therapeutic decision-making based on the FAST results in trauma patients with pelvic fracture.
Descriptive study that includes trauma patients older than 16 with a pelvic fracture admitted to the critical care area or who were fallecimiento. FAST result was compared with a true positive or negative value according to the results of laparotomy or abdominal CT. We recorded diagnosis and treatment of each injury and resolution of the case, detailing the cause of death, among all variables.
Over the 13–year period, we included 263 trauma patients with pelvic fracture, with a mean ISS of 31 and mortality of 19%. FAST had a sensitivity of 65.2%, specificity of 69%, false negative rate of 34.8% and false positive rate of 30.9%. Hemodynamically unstable patients died twice as many stable patients (27% vs. 14%, p < 0.05). Patients with positive FAST died more than negative FAST (43% vs. 26%); and 4 of 10 hemodynamically unstable patients who underwent non therapeutic laparotomy after presenting a false positive FAST died from hypovolemic shock. The mortality rate fell from 60% to 20% when preperitoneal packing was performed before angio-embolization of the pelvis.
FAST has low accuracy in polytraumatized patients with pelvic fracture. Patients with false positive FAST have higher mortality, which can be reduce notably applying a preperitoneal packing.
Diagnosing pulmonary embolisms: the clinician's point of view

La tromboembolia pulmonar (TEP) es una enfermedad frecuente y potencialmente grave. Es necesario el conocimiento de los conceptos básicos que rigen la utilidad de las pruebas para poder realizar una aproximación correcta a su proceso diagnóstico. La TEP se caracteriza porque su diagnóstico es un ejemplo de la aplicación de las probabilidades condicionadas de Bayes a la clínica diaria. Para una correcta interpretación de las pruebas diagnósticas disponibles, es necesario analizar diferentes conceptos que son fundamentales para la toma de decisiones. Es preciso conocer el significado de los cocientes de probabilidad, los intervalos de confianza del 95% y los umbrales de decisión. Así, la utilidad de la determinación del dímero D, o la realización de una angiografía pulmonar mediante tomografía computarizada u otras pruebas de imagen, se debe a la capacidad de modificar la probabilidad pretest de padecer la enfermedad a una probabilidad posterior que se encuentre por encima o por debajo de los umbrales de acción. El objetivo de esta revisión es clarificar la secuencia diagnóstica de la enfermedad tromboembólica pulmonar, analizando las principales herramientas diagnósticas, clínicas, analíticas y de imagen, poniendo especial énfasis en los principios que rigen la medicina basada en la evidencia.
Pulmonary thromboembolism is common and potentially severe. To ensure the correct approach to the diagnostic workup of pulmonary thromboembolism, it is essential to know the basic concepts governing the use of the different tests available. The diagnostic approach to pulmonary thromboembolism is an example of the application of the conditional probabilities of Bayes’ theorem in daily practice. To interpret the available diagnostic tests correctly, it is necessary to analyze different concepts that are fundamental for decision making. Thus, it is necessary to know what the likelihood ratios, 95% confidence intervals, and decision thresholds mean. Whether to determine the D-dimer concentration or to do CT angiography or other imaging tests depends on their capacity to modify the pretest probability of having the disease to a posttest probability that is higher or lower than the thresholds for action. This review aims to clarify the diagnostic sequence of thromboembolic pulmonary disease, analyzing the main diagnostic tools (clinical examination, laboratory tests, and imaging tests), placing special emphasis on the principles that govern evidence-based medicine.
Diagnostic yield of pancreatic percutaneous puncture depending on the size of the lesion

El objetivo del presente estudio fue analizar la eficacia diagnóstica de la punción percutánea de tejido pancreático.
Estudio retrospectivo de pacientes con sospecha de neoplasia de origen pancreático, con biopsia percutánea de tejido pancreático, desde el 2000 hasta el 2011. Para el análisis estadístico comparativo se estratificó la muestra por tamaño, en menores o iguales a 3 cm frente a mayores.
Se realizaron un total de 90 biopsias. Se llegó al diagnóstico de neoplasia pancreática en 47 casos (52%), 16 falsos negativos (18%), 0 falsos positivos y al de pancreatitis crónica en 24 casos (27%). Los resultados de rendimiento de la prueba fueron: sensibilidad (S) global del 75% (intervalo de confianza [IC] 95%: 62-85%), especificidad (E) del 100% (IC 95%: 87-100%), valor predictivo positivo (VPP) del 100% (IC 95%: 92-100%) y valor predictivo negativo (VPN) del 63% (IC 95%: 46-77%). En masas ≤ 3 cm la S fue del 70% (IC 95%: 45-88%), la E del 100% (IC 95%: 66-100%), el VPP del 100% (IC 95%: 76-100%) y el VPN 60% (IC 95%: 32-83%). Frente a masas mayores de 3 cm que presentaron una S del 88% (IC 95%: 70-98%), una E del 100% (IC 95%: 75-100%), un VPP del 100% (IC 95%: 85-100%) y un VPN del 81% (IC 95%: 54-96%).
La rentabilidad de la biopsia percutánea pancreática está fuertemente condicionada por el tamaño de la lesión. Para tamaños tumorales menores de 3 cm la sensibilidad y el valor predictivo negativo son inaceptablemente bajos, por que lo que resultados negativos no serían fiables.
The aim of the present study was to study the diagnostic efficacy of the percutaneous puncture of pancreatic tissue.
A retrospective study was conducted on patients with suspicion of pancreatic neoplasm, and with a percutaneous biopsy of pancreatic tissue, from 2000 to 2011. For the statistical comparative analysis, the sample was stratified by tumour size: ≤ 3 cm and > 3 cm.
A total of 90 biopsies were performed. Pancreatic neoplasm diagnosis was made in 47 cases (52%), with 16 false negatives (18%), no false positives, and chronic pancreatitis in 24 cases (27%). The efficacy of the test results were: an overall sensitivity of 75% (95% CI: 62%-85%), a specificity of 100% (95% CI: 87%-100%), a positive predictive value of 100% (95% CI: 92%-100%), and a negative predictive value of 63% (95% CI: 46%-77%). For tumour sizes ≤ 3 cm the sensitivity was 70% (95% CI: 45%-88%), with a specificity of 100% (95% CI 66%-100%), a positive predictive value of 100% (95% CI: 76%-100%, and a negative predictive value 60% (95% CI: 32%-83%). For tumours greater than 3 cm, the sensitivity was 88% (95% CI: 70%-98%), the specificity was 100% (95% CI: 75%-100%), with a positive predictive value of 100% (95% CI: 85%-100%) and a negative predictive value of 81% (95% CI: 54%-96%).
Pancreatic percutaneous biopsy efficacy was strongly determined by lesion size. For tumour sizes less than 3 cm, the sensitivity and negative predictive value are unacceptably low, as negative results would not reliable.
