Initial experience with hybrid transvaginal cholecystectomy

Case report

Our team comprises general surgeons with experience in advanced laparoscopic surgery. One of the surgeons is also an endoscopist, and another has experience in gynecologic surgery.

The procedure was performed on a 22-year-old woman with symptomatic gallstones. The patient gave us informed consent to perform the surgery, and the procedure was previously approved by our hospital's ethical review board.

The first report of NOTES was published in 2004 when Kalloo et al2 performed a series of acute survival studies at Johns Hopkins University, which demonstrated the feasibility of performing transgastric liver biopsies.     Studies reporting established extraperitoneal endoscopic procedures and those that did not traverse the luminal wall such as abscess or cyst drainage or debridement and transvaginal gynecologic procedures were not included because they are standard practice and are not considered NOTES procedures. The transvaginal approach for resection of the uterus has been used for a long time by gynecologists for hysterectomy, but as of late has been modified by laparoscopic surgeons for the extraction of numerous organs including the gallbladder, appendix, kidney, bladder, and colon.2–4 The vagina has gained the most popularity because it not only is readily accessible and easy to decontaminate but it also provides safe entry and simple closure.
