Analyser les défauts de marche d'un patient hémiplégique est l'étape nécessaire pour proposer une thérapeutique [3], [10]. La gêne provient-elle de l'importance des paralysies, du déséquilibre musculaire, des syncinésies, de la spasticité ou des rétractions ? Disposer d'une technique instrumentale qui soit non invasive, fiable, simple et relativement peu coûteuse en temps et en matériel est le souhait de tout un chacun.
Le système F-Scan (Tekscan ; Inc., South Boston MA) a été développé pour
Patients
La population étudiée comprend 20 patients (13 hommes, sept femmes), présentant une hémiplégie gauche (n = 12) ou droite (n = 8), d'âge moyen 50 ans [25–67 ans].
L'ancienneté de l'hémiplégie était de 1,5 an [2 mois–5,8 ans]. L'hémiplégie était secondaire à un accident vasculaire ischémique (17 patients) ou hémorragique (trois patients). Tous les patients pouvaient marcher seuls, sans aide technique (n = 4) ou avec une canne (simple n = 7, anglaise n = 1, tripode n = 8).

Il existe une symétrie globale du trajet du COP (Fig. 3, Fig. 4) :

la longueur du déplacement postéroantérieur n'est pas significativement différente entre le pied droit et le pied gauche : 15,7 ± 1,1 cm à droite vs 15,9 ± 2,1 cm à gauche ;
- ●
le déplacement du COP se fait du talon vers la partie médiane de l'avant-pied. La progression, qui se fait à vitesse non constante, est régulière et globalement symétrique ;

la trajectoire du COP n'est pas strictement superposable pour les différents pas
Discussion
Il a été montré [11] que les mesures de force avec le F-Scan et avec une plate-forme de force sont fortement corrélées. Il est donc raisonnable d'utiliser le F-Scan qui permet une analyse de la marche dans les conditions habituelles de chaussage.
L'obtention de résultats comparables entre sujets nécessite un protocole strict d'utilisation des semelles embarquées :

semelles adaptées en taille à la chaussure du patient de manière à assurer le meilleur contact possible ;

Conclusion
La baropodométrie par système F-Scan et notamment l'étude de la trajectoire du COP permet l'analyse quantifiée de la dynamique de l'appui du pied au sol dans des conditions de chaussage habituel, ce qui est particulièrement utile chez le patient hémiplégique.
Chez celui-ci, l'asymétrie de roulement du pied au sol est importante, la trajectoire du COP est constamment perturbée : plus courte, plus antériorisée (marge postérieure augmentée), et présentant davantage d'anomalies de progression.
