An anatomical study of the suprascapular nerve

Abstract

The course of the suprascapular nerve and its distance from fixed scapular landmarks were measured in 90 cadaveric shoulders. In an additional 15 cadavers, three pins were passed at various angles in a general anteriorposterior direction through the middle of the glenoid neck just inferior and lateral to the base of the coracoid process. The distance between the exit site on the posterior glenoid neck and the suprascapular nerve at the base of the scapular spine was recorded for each pin. Inferiorly directed pins were the furthest from the suprascapular nerve and averaged 16 mm. On the basis of these data, a relative safe zone is described in the posterior glenoid neck. Knowledge of the anatomic course of the suprascapular nerve may aid the physician in the diagnosis and treatment of suprascapular neuropathies. Appreciation of the safe zone may help the shoulder surgeon avoid iatrogenic injury to the suprascapular nerve during arthroscopic Bankart procedures and other open surgical procedures requiring dissection of the posterior glenoid neck.

Arthroscopic advancement of the supraspinatus muscle and tendon for posterosuperior massive rotator cuff tears

La réparation sans tension des ruptures rétractées du supra-épineux par son avancement musculo-tendineux décrite par Debeyre en 1965 à ciel ouvert, donnait des résultats cliniques satisfaisants. L’objectif de cette étude anatomique était de tester la faisabilité d’une technique arthroscopique d’avancement du supra-épineux. Dix épaules cadavériques ont été opérées. Pour chacune nous avons analysé sa faisabilité, mesuré la distance de translation, et relevé la position du nerf supra-scapulaire (NSS). La réinsertion du tendon sur le footprint natif a été possible dans chaque cas. La distance moyenne de translation était de 38,8 mm ± 3,6 (33–44). Le NSS était libre et sans tension dans toutes les épaules. L’avancement tendino-musculaire du supra-épineux par arthroscopie permet une réparation complète de la coiffe des rotateurs dans les ruptures postéro-supérieures rétractées non réductibles et constitue une alternative intéressante aux autres techniques.

Arthroscopic advancement of the supraspinatus muscle and tendon for posterosuperior massive rotator cuff tears

The tension-free repair of retracted supraspinatus tears with the open muscle and tendon advancement technique first described by Debeyre in 1965 gave satisfactory clinical results. The purpose of this anatomical study was to test the feasibility of an arthroscopic supraspinatus advancement technique. A total of 10 cadaveric shoulders were operated. We assessed the feasibility, measured the slide distance, and recorded the position of the suprascapular nerve (SSN) for each shoulder. Reattachment of the tendon to its native footprint was achieved in all cases. The mean slide distance was 38.8 mm ± 3.6 (33–44). The SSN was released and tension-free in all shoulders. Arthroscopic advancement of the supraspinatus muscle and tendon enables complete rotator cuff repair in irreparable retracted posterosuperior tears and is a noteworthy alternative to the other techniques. Low rates of serious complications after open Latarjet procedure at short-term follow-up

To report on intraoperative and short-term postoperative adverse events after open Latarjet procedure in patients with recurrent anterior shoulder instability. These complications were classified into different grades of severity based on the treatment required and the learning curve of the procedure.
Ninety-six patients (102 shoulders) underwent open Latarjet procedure for recurrent post-traumatic anterior glenohumeral instability between 2012 and 2020. The minimum duration of patients’ follow-up was 6 months. Adverse events were classified into 3 classes based on the severity and subsequent treatment. The complications in the first 50% of all cases were compared with the latter 50% to evaluate the role of learning curve on the complication rates.
The mean follow-up was 7.2 ± 2.8 months. The patients’ mean age was 26.7 ± 8.9 years and consisted of 83 (86.4%) male and 13 (13.6%) female patients. The total adverse events rate was 18.6%. Adverse events requiring no additional treatment (class 1) occurred in 6 cases (5.8%) including fibrous union (3.9%) and asymptomatic resorption of the graft (1.9%). Adverse events requiring additional or extended nonoperative management (class 2) occurred in 8 cases (7.8%), including coracoid fracture (2.9%), musculocutaneous nerve palsy (1.9%), axillary nerve palsy (0.9%), suprascapular nerve palsy (0.9%), and stiffness (0.9%). All the nerve palsies recovered without long-term sequelae. Adverse events requiring secondary operative procedures (class 3) occurred in 5 cases (4.9%), including symptomatic hardware (1.9%), medial healing of the graft (0.9%), screw loosening (0.9%), and deep infection (0.9%). The rate of adverse events in revision cases was higher than primary cases in 11.7% and 6.8%, respectively (P = .119). The complication rate was significantly higher in the first half of the surgeons’ practice (14.7%) than in the second half (3.9%) (P ≤ .05).
The overall complication rate reported in this open Latarjet series is 18.6%; however, the rate of class 3 adverse events that required additional surgery or long-term medical treatment was only 4.9%. Revision cases had a higher rate of complications than primary cases, and the learning curve has had a significant impact on the rate of adverse events.
Two paleopathological cases suggestive of paralabral cysts of the shoulder

To undertake differential diagnosis of scapular bone defects found in two medieval skeletons buried in different necropoles of Friuli Venezia Giulia (Italy) and to evaluate the clinical significance of paralabral cysts in the context of shoulder pathology.
Individual JoT36 is an adult male exhumed from a necropolis belonging to a rural agricultural settlement dated to the 10th to 11th century A.D. Individual CIVT58 is an adult male from a Langobard necropolis dated to 630–670 A.D.
Macroscopic examination of the skeletons was performed using standard osteological methods and review of pertinent clinical literature to assist differential diagnosis.
Between the glenoid cavity and the spinoglenoid notch of the right scapula of JoT36 there is a multilocular circular defect adjacent to the glenoid rim. On the neck of the left scapula of CIVT58, immediately above the spinoglenoid notch, there is a unilocular circular depression. In both cases, the cortical bone appears smooth without any evidence of erosion or sclerosis.
Skeletal findings and historical/archaeological contexts of both cases are compatible with the diagnosis of paralabral cysts.
Paralabral cysts are relatively frequently observed in clinical settings but very few examples have been documented in paleopathological literature. This study seeks to improve recognition and interpretation of this pathology in historical/archaeological contexts.
Findings from a case report can neither generate epidemiological information nor be generalized.
Identification of new cases may add valuable information about lifestyles and related shoulder pathologies in ancient times.
Suprascapular nerve injury secondary to cement extravasation in an anatomic total shoulder arthroplasty: a case report

We report a case of cement extravasation around the suprascapular nerve during an anatomic total shoulder arthroplasty (TSA) in a 68-year-old male with glenohumeral osteoarthritis. The postoperative course was highlighted by shoulder weakness and lower than expected functionality. A shoulder CT scan demonstrated cement extravasation into the suprascapular notch through the superior hole of the pegged all polyethylene glenoid component. This is the first reported case of suprascapular nerve encasement and injury from cement in the literature and highlights the importance of recognizing glenoid vault perforation prior to placing polymethylmethylacrylate cement.
Three-dimensional analysis of baseplate screw penetration in reverse total shoulder arthroplasty: risk of iatrogenic suprascapular neuropathy by screw violation

Baseplate screws have been suggested to be a possible cause of suprascapular neuropathy after reverse total shoulder arthroplasty (RTSA). Three-dimensional analyses of screw penetration and confirmation of its clinical impacts are relevant. The study aims to investigate the association between screw penetration and the clinical outcomes.
Eighty-two patients who underwent RTSA for a massive rotator cuff tear, cuff tear arthropathy, or osteoarthritis with rotator cuff tear were retrospectively enrolled. They were followed up for a minimum of 12 months, and all underwent computed tomography at 1 year postoperatively. The lengths of the superior and posterior baseplate screws were documented. Postoperative computed tomography images were subjected to 3-dimensional analysis to determine whether superior or posterior screws penetrated the glenoid vault and the location to which they penetrated, and screw-to-nerve distances were measured to estimate risks of screw nerve violation and iatrogenic suprascapular neuropathy. Patients with any screw <5 mm from the suprascapular nerve were deemed to have a high risk. Clinical outcomes (functional scores, ranges of motion, and isometric strengths) of patients in the high- and lower-risk groups were compared.
The mean lengths of the superior and posterior screws were 28 ± 4 mm and 18 ± 3 mm, respectively. Penetration was detected for 13% of superior screws and 64% of posterior screws. Sixty-three percent of penetrating superior screws and 5% of penetrating posterior screws were <5 mm from the suprascapular nerve, and therefore, 12% of patients who received RTSA were assessed to have a high risk of iatrogenic suprascapular neuropathy. However, no significant difference was detected in clinical outcomes between the high- and lower-risk patients after a mean follow-up period of 20 months.
Twelve percent of patients who received RTSA were assessed to be at high risk of iatrogenic suprascapular neuropathy by baseplate screw penetration. However, the clinical outcomes of RTSA at a minimum follow-up of 1 year were similar in the high- and lower-risk groups.
