Aim

The outcome of treatment of traumatic cervical spine injuries with autologous bone graft and anterior plate fixation, including injuries with lesions of posterior structures, was studied.

Materials and methods

Retrospective study of 40 patients with traumatic injury of the lower cervical spine (C3 to C7) and a mean follow-up of 2 years. The series consisted of 31 men (77.5%) and 9 women (22.5%), mean age 33 years (15 to 77 years). Patients were treated by reduction and bone fixation with an anterior plate and tricortical iliac crest graft. Residual cervical pain and cervicobrachial pain were assessed clinically on a four-point scale and the final outcome was studied radiographically to analyze bone healing, alignment, and presence of residual listhesis.

Results

Consolidation was achieved in 39 patients (97.5%), with good final alignment of the cervical spine. The clinical outcome was favorable and the complication rate was very low.

Conclusions

Anterior plate fixation is a valid surgical option for traumatic cervical spine injuries, including those that involve posterior structures.
