La publicación del Manual ACR BI-RADS® v2025 representa una evolución significativa respecto a la 5ª edición del Atlas de 2013. Esta actualización responde a la necesidad de modernizar el léxico y adaptar la estructura de los informes ante el avance de tecnologías como la tomosíntesis y la mamografía con contraste, además de la emergencia de nuevos tratamientos definitivos no quirúrgicos. Entre los cambios más destacados se incluye la transición de «Atlas» a «Manual», la adopción de un sistema de versiones basado en el año y la integración completa de la mamografía con contraste como modalidad fundamental. Asimismo, se actualizan las categorías de evaluación, dividiendo la Categoría 0 para alinearse con normativas de la FDA y redefiniendo el manejo de la Categoría 6. Este artículo revisa las modificaciones léxicas y cambios estructurales claves del sistema BI-RADS v2025 para su aplicación en la práctica diaria.

The publication of the ACR BI-RADS® v2025 Manual represents a significant evolution from the fifth edition Atlas of 2013. This update responds to the need to modernise the lexicon and adapt report structure in light of advances in technologies such as tomosynthesis and contrast-enhanced mammography, as well as the emergence of new definitive non-surgical treatments. Among the most notable changes are the transition from ‘Atlas’ to ‘Manual’, the adoption of a year-based versioning system and the full integration of contrast-enhanced mammography as a core modality. Furthermore, assessment categories are updated, splitting Category 0 to align with FDA regulations and redefining the management of Category 6. This article reviews the key lexical modifications and structural changes in the BI-RADS v2025 system for application in daily practice.
