las conductas autolíticas son frecuentes en pacientes que ingresan en una unidad de agudos psiquiátrica. Nuestro objetivo es describir las características clínicas de los pacientes sin diagnóstico de psicosis que tienen conductas autolíticas y compararlos con aquellos que no las tienen.
Métodosla muestra estuvo compuesta por sujetos que durante los años 2022 y 2023 habían ingresado en la Unidad de Agudos del Hospital de Badajoz y que recibían al alta un diagnóstico distinto al de psicosis. Se registraron las conductas autolíticas anteriores al ingreso. Se realizó un estudio prospectivo con un seguimiento de un año.
Resultadosla muestra estuvo compuesta por 298 sujetos. El 71,1% tenían tentativas autolíticas previas. Los que las presentaban respecto a los que no, tenían (variables estadísticamente significativas): más diagnóstico asociado de trastorno de personalidad (82,1% vs. 47,6%; p < 0,001), menos diagnóstico de deterioro cognitivo (5,7% vs. 18,6%; p = 0,014), menos discapacidad reconocida (18,9% vs. 37,2%; p = 0,018), mayor uso de antidepresivos (95,3% vs. 46,5%; p < 0,001) y benzodiacepinas (83% vs. 65,1%; p = 0,017), más trastorno por consumo de sustancias en el pasado (56,6% vs. 30,2%; p = 0,004), más consumo de tabaco en el pasado (66,3% vs. 48,8%; p = 0,047) y mejor conciencia de enfermedad (54,7% vs. 23,3%; p < 0,001).
Conclusioneslos pacientes que ingresaban con un diagnóstico distinto al de psicosis con conductas autolíticas tenían más diagnóstico asociado de trastorno de personalidad, menos diagnóstico de deterioro cognitivo, menos discapacidad reconocida, mayor uso de antidepresivos y benzodiacepinas, más trastorno por consumo de sustancias y de tabaco en el pasado y mejor conciencia de enfermedad.
Suicidal behaviors are common in patients admitted to an acute psychiatric unit. Our objective is to describe the clinical characteristics of patients without a diagnosis of psychosis who exhibit suicidal behaviors and compare them with those without.
MethodsThe sample consisted of subjects who were admitted to the acute psychiatric unit of the Badajoz Hospital during 2022 and 2023 and who received a diagnosis other than psychosis upon discharge. Prior suicidal behaviors were recorded. A prospective study with one year of follow-up was conducted.
ResultsThe sample consisted of 298 subjects. 71.1% had previous suicide attempts. Those who presented them compared to those who did not, had (statistically significant variables): more associated diagnosis of Personality Disorder (82.1% vs 47.6%; p < 0.001), less cognitive impairment (5.7% vs 18.6%; p = 0.014), less recognized disability (18.9% vs 37.2%; p = 0.018), greater use of antidepressants (95.3% vs 46.5%; p < 0.001) and benzodiazepines (83% vs 65.1%; p = 0.017), more past substance use disorder (56.6% vs 30.2%; p = 0.004), more past tobacco use (66.3% vs 48.8%; p = 0.047) and better awareness of illness (54.7% vs 23.3%; p < 0.001).
ConclusionsPatients admitted with a diagnosis other than psychosis with suicidal behavior had a higher rate of associated personality disorders, lower rates of cognitive impairment, lower rates of recognized disability, greater use of antidepressants and benzodiazepines, higher rates of past substance and tobacco use disorders, and better awareness of their illness.
