Resumen
Objetivo

Determinar las positividades mediante técnicas inmunohistoquímicas de la catepsina D (CD) en nuestras muestras así como la intensidad y localización en el espesor del epitelio de los casos positivos para este marcador.

Material y métodos

Nuestro estudio está formado por 113 muestras de tejido cervical divididas en 4 grupos: 30 casos de epitelio escamoso cervical normal (CC), 31 de neoplasia cervical intraepitelial grado 1 (CIN1), 27 de neoplasia cervical intraepitelial grado 2 (CIN2) y 25 de neoplasia cervical intraepitelial grado 3 (CIN3). Se obtienen cortes de tejido, tras su fijación e inclusión en parafina, en los que se aplica una técnica inmunohistoquímica específica para la detección de la CD.

Resultados

La distribución de las positividades de este marcador fueron 0% para el CC, 48% para el CIN1, 33% para el CIN2 y 44% para el CIN3, con p = 0,00001 en la relación CC-CIN1, p = 0,0005 en la relación CC-CIN2 y p = 0,00005 en la relación CCCIN3 (el nivel de significación estadística aplicado ha sido del 5%). La diferencia en la distribución de las intensidades fue significativa en el grupo de tinción intensa: 6,6% para CIN1, 0% para CIN2 y 45% para CIN3 (p = 0,0476).

Conclusión

La expresión de la CD en un alto porcentaje de CIN1 puede atribuirse a su función estimuladora de la mitosis, así como el aumento de intensidad en los CIN3 podría indicar la adquisición temprana de alteraciones propias del fenotipo metastásico. Son necesarios estudios posteriores para sentar como base de la oncogénesis cervical nuestras conclusiones.

Abstract
Objective

The aim of this study was to determine cathepsin D (CD) detection using immunohistochemical techniques in several samples. The intensity and position of stains in epithelial tissue positive for this marker were also analyzed.

Material and methods

This study was carried out in 113 cervical tissue samples, classified in four groups: 30 samples of normal cervical squamous epithelia (CSE), 31 samples of cervical intraepithelial neoplasia grade 1 (CIN1), 27 samples of cervical intraepithelial neoplasia grade 2 (CIN2) and 25 samples of cervical intraepithelial neoplasia grade 3 (CIN3). The samples were fixed and embedded in paraffin and tissue sections were obtained from each. Subsequently an immunohistochemical technique specifically designed for CD detection was applied.

Results

The distribution of positive CD expression was 0% in the CSE group, 48% in CIN1, 33% in CIN2 and 44% in CIN3, with p = 0.00001 for the relation between CC and CIN1, p = 0.0005 for that between CC and CIN2 and p = 0.00005 for that between CC and CIN3 (a significance level of 0.05 was used). Differences in the distribution of intensities was significant for the group showing strong staining: 6.6% for CIN1, 0% for CIN2 and 45% for CIN3 (p = 0.0476).

Conclusion

Positive CD detection in a high fraction of CIN1 could be related to its function of mitogenic activity stimulation. Similarly, the higher intensity values in CIN3 could well indicate early detection of changes due to metastatic capacity. Further studies are required to confirm that this mechanism underlies malignant transformation of the ectocervical squamous epithelium.
