Abstract
Objective

a) To describe the final outcome of in vitro fertilization/intracytoplasmic sperm injection cycles and the incidence and severity of ovarian hyperstimulation syndrome (OHSS) in patients undergoing coasting, due to a high response to controlled ovarian hyperstimulation (COH); and b) To determine whether the duration of coasting affects the final cycle outcome

Patients and methods

Retrospective study of 42 in vitro fertilization/intracytoplasmic sperm injection cycles in patients with a high response to COH and submitted to coasting (withholding gonadotrophins while maintaining gonadotrophinreleasing analogue), until serum estradiol (E2) level lowers to a “safe” value allowing administration of human chorionic gonadotrophin and ovarian puncture

Results

Mean serum E2 level at the beginning and at the end of coasting was 6,427.6 ± 227.4pg/ml and 2,454.7 ± 155.8pg/ml respectively. The implantation and pregnancy rates were 26.9% and 53.8%, respectively. None of the patients had severe OHSS and only 14.2% had moderate OHSS. In patients in whom the coasting was prolonged for > 4 days, the pregnancy rate was statistically significantly lower than in those in whom it lasted for ≤ 4 days (28.6% versus 57.9%; p<0.05). No difference was found in the incidence of OHSS between both groups

Conclusions

Coasting is effective in reducing the incidence and severity of OHSS. This procedure prevents cicle cancellation in patients at high risk for developing OHSS, and achieving an excellent pregnancy rate. The results of coasting are better if it is not prolonged for more than 4 days
