El diagnóstico de lesión intracraneal (LI) tras traumatismo craneoencefálico (TCE) leve se realiza mediante tomografía computarizada (TC) craneal, técnica que implica radiación y tiempos de espera en urgencias. Existen biomarcadores séricos, como S100β, GFAP y UCH-L1, que podrían ser alternativas rápidas y eficientes para descartar LI.
Material y métodosEstudio observacional transversal de evaluación de pruebas diagnósticas en pacientes adultos atendidos en urgencias entre 2023 y 2024 por TCE leve, en quienes se realizó TC craneal y determinación sérica de estos biomarcadores.
ResultadosSe incluyeron 329 pacientes con edad media de 77 años; el 59,3% eran mujeres. El 8,4% mostró LI en la TC craneal. La sensibilidad fue del 92,9% y el valor predictivo negativo (VPN) del 96,3% en el TBI (ensayo cualitativo derivado de la interpretación conjunta de GFAP y UCH-L1), superior al del S100β. Los pacientes con LI presentaron concentraciones más elevadas de GFAP.
ConclusionesEl ensayo TBI y, más concretamente, el marcador GFAP mostró mayor rendimiento diagnóstico, apoyando su uso como herramienta para optimizar la indicación de TC craneal en urgencias tras un TCE leve.
The diagnosis of intracranial injury (ICI) after mild traumatic brain injury (TBI) is made using cranial computed tomography (CT), a technique that involves radiation and waiting times in the emergency department. Serum biomarkers, such as S100β, GFAP, and UCH-L1, could be rapid and efficient alternatives for ruling out ICI.
Materials and methodsThis was a cross-sectional observational study evaluating diagnostic tests in adult patients treated in the emergency department between 2023 and 2024 for mild TBI, who underwent cranial CT and serum determination of these biomarkers.
ResultsA total of 329 patients with a mean age of 77 years were included, 59.3% of whom were women. 8.4% showed ICI on cranial CT. Sensitivity was 92.9% and negative predictive value was 96.3% in TBI test (GFAP+UCH-L1), higher than that of S100β. Patients with ICI had higher GFAP concentrations.
ConclusionsTBI test, and more specifically GFAP, showed greater diagnostic performance, supporting its use as a tool to optimize the indication for cranial CT in the emergency department after mild TBI.
