Abstract
Introduction

Overweight is a major public health problem, which involves different factors. The aim of this study is to analyze the evolution of the prevalence of overweight and identify changes in its trend in Spain and its autonomous communities in a population aged 15 years and older from 1987 to 2023.

Methods

Data was obtained from the Encuesta Nacional de Salud de España (ENSE/ESdE) and the Encuesta Europea de Salud en España (EESE), from 1987 to 2023. Prevalence of overweight for Spain and its autonomous communities was estimated overall, by sex and age group with their 95% confidence intervals (95%CI). Relative changes in prevalence are presented and trend analysis was conducted using joinpoint regression models. Annual percentage change (APC) and its 95% confidence interval (95%CI) was estimated.

Results

The prevalence of overweight increased from 32.8% (95%CI 32.2–33.5%) in 1987 to 38.9% (95%CI 38.0–39.7%) in 2023. The prevalence was higher in men during the entire period, reaching 45.8% in 2023. In women, the greatest relative change was observed among women aged 15–24 years (113.1%); also, the trend analysis indicated an annual increased in overweight prevalence of 1.6% since 2014. Canarias and Murcia stand out for presenting a consistently increasing trend of overweight throughout the period.

Conclusion

The findings of this study indicate that the prevalence of overweight remains considerably high, being higher in men and has increased in the period studied, particularly in women and in the younger population. The results suggest that public health strategies should be implemented at the population level, with particular emphasis on groups identified as vulnerable.
