Implementar PrEP y PPE en mujeres cis y trans trabajadoras sexuales es un reto; es un colectivo infrarrepresentado en programas preventivos en España.
MétodosEstudio descriptivo retrospectivo de mujeres cis y trans trabajadoras sexuales sin VIH atendidas en un centro de Madrid (diciembre 2024-agosto 2025; n=34).
ResultadosEl 85,3% eran mujeres cis (mediana de edad: 28 años), el 85,3% de origen latinoamericano y el 88,2% en situación administrativa irregular. La prevalencia basal de ITS fue del 32,4%, con bajo cribado previo (20,6%). Recibieron PPE 16 pacientes (47,1%), transitando 9 a PrEP. Iniciaron PrEP 17 pacientes (50,0%); de ellas, 6 (35,3%) discontinuaron, principalmente por intolerancia digestiva en las primeras semanas. Se vacunaron 24 pacientes (70,6%).
ConclusionesLa PPE se asoció a una notable transición a PrEP. La alta discontinuación temprana subraya la necesidad de optimizar el seguimiento inicial y plantea las formulaciones inyectables como alternativa estratégica en esta población vulnerable.
Implementing PrEP and PEP among cis and trans female sex workers remains a challenge, representing an underrepresented population in prevention programs in Spain.
MethodsRetrospective descriptive study of HIV-negative cis and trans female sex workers managed at a Madrid clinic (December 2024-August 2025; n=34).
ResultsMost (85.3%) were cis women (median age: 28 years); 85.3% were of Latin American origin and 88.2% had an irregular administrative status. Baseline STI prevalence was 32.4%, with low prior screening (20.6%). PEP was received by 16 patients (47.1%), and 9 transitioned to PrEP. PrEP was initiated by 17 patients (50.0%); 6 (35.3%) discontinued, primarily due to gastrointestinal intolerance within the first weeks. Vaccination reached 70.6% (24 patients).
ConclusionsPEP was associated with significant PrEP uptake. High early discontinuation rates highlight the need to optimize follow-up and suggest long-acting injectable formulations as a strategic alternative for this vulnerable population.
