Se ha realizado un estudio retrospectivo con el objetivo de valorar la respuesta bioquímica [GH e IGF-1] en un grupo de pacientes con acromegalia que recibieron tratamiento con análogos de la somatostina, en función de la respuesta obtenida frente a diversas pruebas de estimulación hipofisaria. Se estudió a 21 pacientes acromegálicos de los cuales 8 eran varones y 13 mujeres, de edades comprendidas entre 22 y 73 años. Los pacientes fueron tratados con lanreótida por vía intramuscular a dosis de 30 mg cada 14 días. La evaluación clínica y bioquímica de la respuesta al tratamiento se realizó a los 3 meses de iniciado el mismo. Antes de iniciar el tratamiento se realizaron tests de estimulación hipofisaria (GHRH, TRH, GnRH y sobrecarga oral de glucosa). Diez de los 21 pacientes estudiados (47,6%) alcanzaron concentraciones de GH inferiores a 2 ng/ml, 15 (71,4%) normalizaron las concentraciones de IGF-1 para su edad y sexo, y 10 (47,6%) lograron alcanzar ambos criterios. No se encontraron diferencias significativas en la consecución de los criterios bioquímicos entre los pacientes con respuesta paradójica o no a TRH, GnRH o sobrecarga oral de glucosa, ni entre los que presentaban o no respuesta a GHRH. Las estimulaciones con GHRH, TRH o GnRH no tuvieron valor para predecir qué pacientes normalizarían las concentraciones de GH e IGF-1.

The aim of the present study was to evaluate the biochemical response to somatostatin analogs in acromegalic patients according to the response obtained after different pituitary stimulation tests. Twenty one patients were studied (8 men and 13 women) with age of 22 to 73 years. Patients were treated with intramuscular lanreotide a dose of 30 mg every 14 days. Biochemical response evaluation was done 3 months after the begining of treatment. Before begining lanreotide, pituitary stimulation tests were performed (GHRH, TRH, GnRH and oral glucose tolerance test). Ten out of the 21 patients studied (47.6%) reached a level of GH lower than 2 ng/ml, 15 (71.4%) reached normal levels of IGF-1 according to age and sex and 10 (47.6%) reached both criteria. No significant differences were found about reaching the biochemical criteria between those patients with different responses to stimulatory tests. We therefore conclude that pituitary stimulatory tests are not useful to predict which patients will normalize GH and IGF-1 after somatostatin analogs treatment.
