The patient is a 63-year-old male with previously diagnosed Crohn’s disease. He came to the ER reporting fever, pain, and purulent discharge from the lumbosacral region. He presented multiple enterocutaneous fistulae and an exophytic lesion (Fig. 1). The patient underwent elective surgery to create a colostomy, and treatment was established with adalimumab and azathioprine. Biopsy of the exophytic lesion identified an epidermoid carcinoma of the skin.

Diagnosis: Complicated Crohn’s disease with enterocutaneous fistulae and malignant degeneration of the skin.
