La poliangeítis granulomatosa es una patología sistémica autoinmune caracterizada por la afectación renal, respiratoria y vascular en forma de vasculitis necrotizante granulomatosa, cuya edad típica de presentación es la quinta y sexta década de la vida, siendo infrecuente su presentación localizada sin afectación sistémica. Presentamos el caso de una paciente de 10 años con un cuadro de edema palpebral recurrente como clínica inicial en el seno de una granulomatosis con poliangeítis (GPA) sistémica.
Aunque infrecuente, esta enfermedad puede afectar a pacientes pediátricos, siendo en ocasiones un reto diagnóstico dada su escasa incidencia y forma de presentación atípica, como es el caso que presentamos.
Granulomatosis with polyangiitis is a systemic autoimmune disease characterized by the presence of granulomatous necrotizing vasculitis with kidney, respiratory, and vascular involvement, whose peak incidence is in the fifth and sixth decades of life. Its localized presentation without systemic involvement is infrequent. We present the case of a 10-year-old patient with recurrent eyelid edema as the initial symptom in the setting of granulomatosis with systemic polyangiitis.
Although rare, this disease can affect children, as in the case we present, and given its low incidence and atypical presentation it is sometimes a diagnostic challenge.
