Medir la repetibilidad del registro de ciclotorsión estática mediante la plataforma de láser excímer WaveLight™ EX500 y su fiabilidad en términos del porcentaje de errores durante las diferentes fases de la cirugía.
Material y métodosEstudio experimental, prospectivo y descriptivo. Se evaluó a los candidatos a cirugía LASIK con un topógrafo basado en discos de Plácido para el registro del iris. Ya en quirófano y tras colocar el blefarostato (Fase 1), se realizaron 3 intentos consecutivos de medición de ciclotorsión con el láser excímer. Se anotaron las mediciones válidas o el número de errores. Posteriormente, tras la creación del flap (Fase 2) y tras el levantamiento del flap (Fase 3), se registró la primera medición válida de ciclotorsión, con un máximo de 3 intentos. Se calculó el número y porcentaje de errores en cada fase y el coeficiente de correlación intraclase (CCI).
ResultadosSe incluyeron 135 ojos de 80 pacientes. Se observaron errores consecutivos en las 3 mediciones en el 2,9, 20,7 y 23% de los ojos en las Fases 1, 2 y 3, respectivamente. El CCI entre las 3 mediciones fue de 0,960. La diferencia máxima entre las mediciones de las Fases 1, 2 y 3 fue de 5,5°. El 20% de los pacientes con una medición válida en las Fases 1 o 2 no logró una medición válida en la Fase 3.
ConclusionesLas mediciones de ciclotorsión estática son altamente repetibles. Es más probable que ocurran errores de registro tras el levantamiento del flap.
To measure the repeatability of static cyclotorsion registry by the WaveLight™ EX500 excimer laser platform and its reliability in terms of the percentage of errors during different phases of the surgery.
Material and methodsExperimental, prospective, descriptive study. Candidates for LASIK surgery were evaluated with a Placido-based topographer for iris registration. Once in the operating room and after placing the blepharostat (phase 1), 3 consecutive cyclotorsion measurement attempts were performed using the excimer laser. Valid measurements or the number of errors were noted. Then, after the flap creation (phase 2) and after the flap lifting (phase 3) the first valid cyclotorsion measurement up to a maximum of three attempts was noted. The number and percentage of errors in every phase were calculated. The intraclass correlation coefficient (ICC) for absolute agreement was also calculated.
ResultsOne hundred thirty-five eyes of 80 subjects were included. Three consecutive measurement errors were found in 2.9, 20.7, and 23% of the eyes in phases 1, 2, and 3, respectively. In Phase 1, the mean range of the 3 measurements was 0,7° with a maximum difference of 3.5°. The ICC between the 3 measurements was 0.960. The maximum difference between the measurements of Phases 1, 2, and 3 was calculated as 5.5°. Twenty percent of patients with a valid measurement in Phases 1 or 2 were unable to achieve a valid measurement in Phase 3.
ConclusionsStatic cyclotorsion measurements are highly repeatable. Registry errors are more likely to occur after flap lifting.
