Se presentan cinco casos de pulmón hiperclaro en los que se comprueba la variada etiología y afectación clínica en este síndrome. Aunque el grado de afectación funcional encontrado fue muy variable, el patrón de restricción espirométrica con atrapamiento aéreo, disminución de TLCO con KCO proporcionalmente más conservada e importante diferencia entre TLC (Pie) y TLC (He) son hallazgos constantes en nuestros casos. Encontramos frecuente asociación de este síndrome con afectación difusa de la via aérea. Se comentan las actitudes diagnósticas y terapéuticas a seguir en este síndrome; comunicando un caso en que la neumonectomía de un pulmón quistico, que limitaba la expansión del pulmón contralateral, fue seguida de una notable mejoría clínica y funcional.

Five cases of hyperlucent lung (HL) illustrating its multiple causes and types of clinical presentation are reported. Although the degree of functional impairment found was very variable, the pattem of spirometric restriction with air trapping, the reduction in TLCO with proportional preservation of KCO, and the significant difference between TLC (Ple) and TLC (He) are constant finding in our cases. We have found a frequent association of this syndrome with diffuse airways obstruction. Guidelines for diagnostic and therapeutic management of this syndrome are commented upon, and a case where resection of a cystic lung limiting contralateral lung expansion was followed by a remarkable clinical and functional improvement is reported.
