A propósito de un caso clínico, se comenta la aspergilosis pulmonar invasiva y su escasa frecuencia en personas aparentemente sanas, así como los pasos que llevan al diagnóstico y los aspectos más importantes del tratamiento.

Como conclusión, insistimos en la necesidad de pensar en esta entidad, no sólo en pacientes inmunodeprimidos, sino también en aquellos que presentan fiebre e infiltrados pulmonares que no responden adecuadamente a los antibióticos. Particularmente, cuando se detectan aspergillus en los estudios microbiológicos, aunque aparentemente su respuesta inmune sea normal.

The aulhors present a case of invasive pulmonary aspergillosis, an infectious disease but rarely seen in apparently healthy individuals. A discussion of the diagnostic and therapeutic features is included and it is recommended that the entity should be suspected when patients with fever and pulmonary infiltrates —whether immuno-suppressed or not— show a poor response to antibiotic therapy and in particular when aspergillus organisms are found in cultures even thought the immune response is apparently normal.
