Alveolitis fibrosante debutando como infiltrado pulmonar con eosinofilia periférica

Se comunica el caso de una mujer de 36 años que presentaba tos, febrícula y disnea de esfuerzo. En la exploración física tenía crepitantes en ambas bases pulmonares. Analíticamente destacaban una elevación de la enzima lactato deshidrogenasa y de la cifra de eosinófilos circulantes (13.000/mm3). En la radiografía de tórax existía un patrón intersticial reticular en ambas bases pulmonares. El lavado broncoalveolar reveló una elevación de los neutrófilos y eosinófilos (32% macrófagos, 22% neutrófilos, 45% eosinófilos). La biopsia pulmonar confirmó el diagnóstico de alveolitis fibrosante. Se descartaron otras causas de eosinofilia periférica. Este caso describe la asociación, no descrita, de eosinofilia periférica y alveolitis fibrosante.

This study reports the case of a 36 years old woman who presented cough, febricula and dyspnea to exertion. Physical examination revelated basa] rales in both lungs. Laboratory examination showed a raised lactic acid dehydrogenase and an increased count of eosinophile blood cells (13.000/mm3). Chest x-ray radiography evidenced a reticular interstitial pattern in both pulmonary basal regions. Bronchoalveolar aspirate showed increased number of neutrophile and eosinophile cells (32% of macrophages, 22% of neutrophiles, and 45% of eosinophiles). Pulmonary biopsy confirmed the diagnosis of fibrotic alveolitis. Other causes of peripheral eosinophilia were rouled out. The clinical case shows the non reported association of peripheral eosinophilia and fibrotic alveolitis.
