Summary
Introduction

Gluteal aneurysm, whether true or false, are rare and mostly are caused by pelvic fractures or penetrating injuries.

Case report

We report a case of a 66-year-old-man who suffereda penetrating wound to the left buttock. Wound was sutured in his regional hospital. On post operative, the patient experienced left sciatic pain and paresis, and the electro physiologic studies revealed compression of the sciatic nerve. Four weeks later the pain continued, and the nuclear magnetic resonance examination revealed apseudoaneurysm of the left internaliliac artery. The patient was admitted in our Vascular Service, the physical examination revealed a pulsatile mass over the left buttock. The duplex ultrasonography, and selective angiography demonstrateda superior gluteal artery pseudoaneurysm. The gluteal pseudoaneurysm was successfully treated by selective embolization, and the patient was free of neurologic symptoms. The presentation and management of patients with gluteal aneurysm are discussed, and the literature reviewed.

Introducción. Los aneurismas, verdaderos o falsos, de la arteria glútea superior o inferiorson muy raros y la mayoríase relacionan con traumatismo pélvico o herida penetrante en la región glútea.

Caso clínico

Presentamos el caso de un varón de 66 años que sufrió una herida penetrante en la región glútea izquierda,que inicialmente se reparó quirúrgicamente en su hospitalde referencia. En elpostoperatoriopresenta dolor ciáticoy paresia de la extremidad; se realizan electromio gramay electron euro grama, que son compatibles con lesión del nervio ciático sin sección completa. Porpersistencia del dolor, cuatro semanas después se realiza resonancia magnética nuclear, que demuestra pseudoaneurisma dependiente de la arteria hipogástrica. Ante este hallazgo, se remite a nuestro Servicio de Angiología y Cirugía Vascular, donde se comprueba, porla exploración física, masapulsátil en región glútea. Se complementa el diagnóstico con ecografía Doppler, y la arteriografía demuestra que el falso aneurisma depende de la arteria glútea superior. Durante laprueba arteriográfica se realizaembolización selectiva de laarteriaglútea superior con éxito, con mejoría de la sintomato-logía neurológica; se da de alta alpaciente a las 72 horas. Se discuten las formas de presentación, las opciones terapéuticas ante los aneurismas de la arteriaglútea y se revisa la literatura actual.

Introdução. Os aneurismas, verdadeiros oufalsos, da artéria glútea superiorou inferior,são muito raros e a maioria está relacionadacom traumatismo pélvico ouferida penetrante na região glútea.

Caso clínico

Apresentamoso caso de um homem de 66anos que so freu ferida penetrante da região glútea esquerda que inicialmente foi reparada cirurgicamente no hospital da sua zona de residência. No pós-operatório apresenta dor ciática e paresia da extremidade, realizando-se electromiografia e electroneuro-grafia que são compatíveis com lesão do nervo ciático sem secção completa. Por persistência da dor, quatro semanas depois, realiza-se ressonância magnética nuclear, que demonstra pseudoaneurisma dependente da artéria hipogástrica. Perante este achado, é remetido ao nosso Serviço de Angiologia e Cirurgia Vascular, onde se comprova por exploração física, massa pulsátil na região glútea. Complementa-se o diagnóstico com eco-Doppler e a arteriografia que demonstra falso aneurisma dependente da artéria glútea superior. Durante a arteriografia, realiza-se com êxito embolização selectiva da artéria glútea superior, com melhoria da sinto-matologia neurológica, sendo-lhe dada alta as 72 horas. Conclusão. Discutem-se as formas de apresentação das opções terapêuticas perante os aneurismas da artéria glútea e revê-se a literatura actual.
