Summary
Introduction

Up to 20% of in situ bypass grafts (BP) develop a hemodynamic failure (HF) in their follow-up, the management of which is still controversial.

Objective

Evaluation of the results of BP revised due to HF.

Patients and methods

Forty-seven BP were revised because of severe stenosis in the anastomosis or the midgraft (1985-2001). Diagnosis: clinical, hemodynamic, ecographic. Location of the defect: proximal anastomosis (19.1%), midgraft (42.6%) and distal anastomosis (38.3%). Seventy percent of the reinterventions were performed within the first year after the initial surgery, 13% in the second year, 17% later. Six cases were repaired through a percutaneous transluminal angioplasty (PTA) and 41 cases surgically.

Results

Medium follow-up after repair: 35 months. Assisted-primary patency (APP): 95.7% after a month, 84.2% after a year and 72% after 3 years. Event-free percentage (EFP): 95.7, 70.5 and 53.2% respectively. Limb salvage (LS): 100, 97.5 and 86.7%. The location of the defect or the distal anastomosis and the graft diameter have had no significant influence on the results (p< 0.05). The PTA has increased the risk of late occlusion (relative risk [RR]= 12.9; confidence interval [CI] 95%= 2.8-60;p= 0.002) and amputation (RR= 14.3; CI 95%= 1.3-157; p= 0.028). Reintervention within the first year has increased the risk of developing a new HF in the follow-up (RR= 3.8; CI 95%= 1.1-16.8; p= 0.034).

Conclusions

We obtained good APP and LS after revision of failing grafts, although they require careful surveillance as it is not unusual to encounter new events during the follow-up. Surgical treatment is effective. We recommend indefinite follow-up because of possible HF beyond the first year.

Introducción

Hasta en un 20% de las derivaciones in situ (BP) aparece un fracaso hemodinámico (FH) en su seguimiento, además de que existe aún controversia acerca de su manejo.

Objetivo

Evaluar los resultados de BP reintervenidos por FH.

Pacientes y métodos

Se han reintervenido 47 BP por estenosis grave en el trayecto o las anastomosis (1985-2001). Diagnóstico: clínico, hemodinámico, ecográfico. Localización de la lesión: anastomosis proximal (19,1%), trayecto del injerto (42,6%), anastomosis distal (38,3%). El 70,2% de las reintervenciones se realizaron dentro del primer año tras la intervención inicial, el 13% en el segundo año y un 17% más tarde. Se repararon seis casos mediante angioplastia percutánea (PTA) y 41 mediante cirugía.

Resultados

Seguimiento medio tras la reparación: 35 meses. Permeabilidad primaria-asistida (PPA): 95,7% al mes, 84,2% al año y 72% a los tres años. Porcentaje libre de eventos (PLE): 95,7, 70,5 y 53,2% respectivamente. Salvación de la extremidad (SE): 100, 97,5 y 86,7%. La localización del defecto o de la anastomosis distal y el diámetro del injerto no han influido en los resultados (p< 0,05). La PTA ha aumentado el riesgo de oclusión (riesgo relativo [RR]= 12,9; intervalo de confianza [IC] 95%= 2,8-60; p= 0,002)y amputación tardías (RR= 14,3; IC 95%= 1,3-157; p= 0,028). La reintervención dentro del primer año ha aumentado el riesgo de nuevo FH en el seguimiento (RR= 3,8; IC 95%= 1,1-16,8;p= 0,034).

Conclusiones

Se consiguen buenas PPA y SE tras la reparación de injertos en riesgo, aunque requieren una vigilancia estrecha por no ser rara la presentación de nuevos episodios en su seguimiento. El tratamiento quirúrgico es eficaz. Es recomendable el seguimiento indefinido por posible FH después del primer año.

Até 20% dos by-passes in situ (BP) apresentam fracasso hemodinâmico (FH) no seu seguimento, para além de que ainda existem controvérsias acerca do seu tratamento.

Objectivo

Avaliar os resultados do BP repetidos por FH.

Doentes e métodos

Foram resubmetidos a cirurgia 47 BP por estenose aguda do trajecto ou das anastomoses. (1985-2001). Diagnóstico: clínico, hemodinâmico, ecográfico. Localização da lesão: anastomose proximal 19,1%; porção do enxerto 42,6%; anastomose distal 38,3%. 70,2% das repetiçõs realizaram-se dentro do primeiro ano, após a cirurgia inicial, 13% no segundo ano, 17% mais tarde. Seis casos foram reparados por angioplastia percutânea (PTA) e 41 por cirurgia.

Resultados

Seguimento médio após a reparação: 35 meses. Permeabilidade primária assistida (PPA): 95,7% ao mês, 84,2% ao ano e 72% aos três anos. Percentagem isenta de eventos (PLE): 95,7, 70,5 e 53,2%, respectivamente. Recuperação daextremidade (RE): 100, 95,7 e 86,7%. A localização do defeito ou da anastomose distal e o diâmetro do enxerto não influíram nos resultados (p< 0,05). A PTA aumentou o risco de oclusão (risco relativo, RR = 12,9; intervalo de confiança, IC 95%= 22,8-60; p= 0,002) e amputação tardias (RR= 14,3; IC 95%= 1,3-157; p= 0,028). A reintervenção dentro do primeiro ano aumentou o risco de novo FH no seguimento (RR= 3,8; IC 95%= 1,1-16,8; p= 0,034).

Conclusões

Conseguem-se boas PPA e RE após a reparação de enxertos em risco, embora exijam uma vigilância estreita por não ser rara a apresentção de novos eventos no seu seguimento. O tratamento cirúrgico é eficaz. Recomenda-se o seguimento indefinido por possível FH após o primeiro ano.
