Although PCa presents as an aggressive and deadly disease, many prostate adenocarcinomas are indolent, with a low probability of ever causing symptoms or death.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> In fact, while PCa incidence has been increasing over the previous decades, low-risk adenocarcinomas are ever more frequent.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> For several years, elderly or frail patients with limited life-expectancy from competing conditions, have been managed conservatively. The main principle of this form of conservative management, called Watchful Waiting (WW), is that the patient’s PCa is expected to have a lower impact on his life-expectancy than his other comorbidities. Therefore, any radical treatment with curative intent is suspended in order to avoid unnecessary side-effects that do not improve patient life-expectancy, acknowledging that age or competing non-prostatic diseases, in some cases, are the principal determinants of demise. Patients managed with WW are monitored for the presence of symptoms and treatment may be considered in order to palliate symptoms deriving from disease progression.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">However, this form of treatment deferment and close monitoring has become increasingly frequent over the previous years. Active surveillance (AS) is a form of conservative management that aims to reduce over-treatment for low-risk PCa patients. In order to consider AS it is necessary to confirm there is no co-existent higher-grade cancer, often in the form of multiparametric magnetic resonance imaging (mpMRI) and confirmatory prostate biopsy. Furthermore, during AS, the patient is periodically monitored for grade or stage progression, using DRE, PSA, mpMRI and repeat biopsy, in order to allow the beginning of active treatment if necessary, without compromising patient life-expectancy.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a> AS was first described in 2002 in which patients in the low-risk category were then managed with periodic prostate biopsies and serial PSA testing only.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">While WW may also be suited for low risk patients, they will never benefit from an aggressive treatment course due to limited life expectancy from competing causes. Management is mainly symptom-guided in the case of disease progression and local treatment is no longer an option.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Over the past several years, the experience with AS has grown considerably, and AS is now standard in countries like Australia, USA or Sweden.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Despite the growing body of evidence on the safety of AS, there is still no standard protocol for AS that is broadly accepted on a multicentric or multinational level.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Material and methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Survey</span><p id="par0020" class="elsevierStylePara elsevierViewall">A feasibility study was conducted among a random sample (n = 28) of urologists working in different settings: university hospital, community hospital and private practice. After analyzing the results from the preliminary survey, the main outcome questions were defined and the definitive survey was elaborated and disseminated. All participants surveyed were practicing urologists in Portugal.</p><p id="par0025" class="elsevierStylePara elsevierViewall">A nationwide anonymous e-survey concerning habits and practices on AS of PCa was then conducted among Portuguese urologists. This web-based survey consisted of twelve questions and was sent electronically to all 368 current members of the Portuguese Urological Association (APU). It was generated and managed online using Google Forms software. The questionnaire gathered information regarding demographics, current and recent changes in AS usage, opinions on patient selection and barriers to implementation of AS. The aim of this preliminary questionnaire was to assess the perceptions and attitudes of Portuguese urologists towards AS and other treatment recommendations for low-risk PCa in their Urology workplace setting.</p><p id="par0030" class="elsevierStylePara elsevierViewall">An accompanying email with a brief explanation of the study and the link/invitation to complete the survey was sent on May 2019, and two similar reminder emails in June 2019 were sent for those who had yet to complete the survey. Respondents were required to answer all questions for submission of their survey. Each respondent could only submit a single survey, using their email account and all responses were confidential. The twelve questions pertained to respondents’ professional setting and their clinical rationale were analyzed for statistical association with their treatment recommendations. The survey contained screening questions to ensure respondents were currently practicing and not in training, in order to exclude Urology residents from participating.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">The primary outcomes of the survey were attitudes toward AS, frequency of use of AS in clinical practice and difficulties of AS from the clinical scenarios’ constraints, obtained through survey responses on the use of AS. Pearson chi-square test was used to assess differences in perceptions of AS and treatment recommendations by physician years of practice. SPSS v26.0 (IBM) was used to perform all statistical analysis. Descriptive statistics was used to characterize the respondents and survey responses (ver material adicional).</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">Overall, from a total of 368 surveys sent, 56 Portuguese Urologists completed the surveys, 49 males (87.5%) and 7 females (12.5%) corresponding to a 15.21% answer rate. Urologists were evenly distributed from a geographic point of view, with 33.9% from the northern part of mainland Portugal, 23.2% from the Center, 39.3% from the South and 3.6% from Portugal’s two archipelagos. Respondents were allocated according to years of experience as practicing urologist and also on the number of PCa patients managed, on average, every month (considering that most Portuguese urologists manage other urologic diseases as well). The study was well balanced for years of experience, with 50% of respondents (n = 28) having more than ten years of experience as a consultant urologist, while the other half less than/or 10 years of experience. Only a fifth of respondents deal with less than/or 20 PCa cancer patients every month, while the vast majority consult more then 20 PCa cancer patients a month (78.6%). Most (n = 26) of the respondents worked at a community hospital (46.4%), 18 (32.1%) at a university hospital and 12 (21.4%) at a private hospital (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">The vast majority (over 80%) of respondents, recommends AS to their patients, with only 5% answering that they do not even approach the subject. The remaining 15% of respondents discuss AS it in the end and doesn’t recommend it (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">When asked to choose the preferred age on which they recommend AS, close to a third believe that age is not a factor while another third chooses to recommend it for patients over 70 years of age. In relation to patient selection (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>): <ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">-</span><p id="par0055" class="elsevierStylePara elsevierViewall">100% of respondents agree that AS is adequate for ISUP grade 1 patients;</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">-</span><p id="par0060" class="elsevierStylePara elsevierViewall">Close to 90% of respondents choose patients for AS whose PSA serum level is bellow 20 ng/mL;</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">-</span><p id="par0065" class="elsevierStylePara elsevierViewall">Close to 80% of respondents state that the number of positive cores is an important factor and 58,5% refer that the percentage of core involvement is also significant.</p></li></ul></p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">Close to 70% of respondents assumed that observance of AS programs by patients was not in question (in spite of the frequent need for redo biopsy) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). When questioned to elicit what were the key limitations of an AS program:<ul class="elsevierStyleList" id="lis0010"><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">-</span><p id="par0075" class="elsevierStylePara elsevierViewall">94.3% of respondents believed overall survival didn’t change when choosing deferred treatment over immediate treatment;</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">-</span><p id="par0080" class="elsevierStylePara elsevierViewall">54.7% of respondents’ opinion was that harboring an oncological disease without local treatment constituted an important and harmful psychological morbidity;</p></li><li class="elsevierStyleListItem" id="lsti0030"><span class="elsevierStyleLabel">-</span><p id="par0085" class="elsevierStylePara elsevierViewall">From experience, 52.8% of respondents stated that the patients were not keen on AS and choose other treatment methods.</p></li></ul></p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">We also noted that most respondents (79.2%) believed that surveillance protocols already in place, as per international guidelines, were adequate and sufficient, and 49.1% alleged that the major difficulties were the availability and constraints to provide for periodic mpMRI and re-biopsy testings. Finally, when inquired about patient preference regarding localized PCa treatment, respondents state that 52.8% prefer surgical treatment and 37.7% choose radiotherapy, with only 9.4% choosing AS.</p><p id="par0095" class="elsevierStylePara elsevierViewall">When accounting for years of experience, and preferences accordingly, there is a bigger concern with histopathology factors while other considerations are almost the same (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">When we related AS favoring according to years of experience (over and under 10 years of experience), geographical location, workplace setting (university hospital, private practice or community hospital) and PCa patient volume (under 20 patients a month, 20–50 patients a month, over 50 patients a month) we only found a statistical significance for workplace setting (p = 0.047) (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>).</p><elsevierMultimedia ident="tbl0030"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0105" class="elsevierStylePara elsevierViewall">To our knowledge, this is the first national, urologist-based study concerning deferred treatment in Portugal, revealing important details about the use of AS in men with localized PCa in different settings. It sought to measure the usage of AS and more specifically, urologist implementation of AS into their clinical practice as well as difficulties that limited it widespread use. PCa management trends published in the literature (with the notable exception of studies like CaPSURE, from Cooperberg et al.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> are usually the result of outcomes achieved in academic centers, where data control and follow-up are more thorough and sometimes the specific focus of clinical trials. However, these outcomes may not extrapolate faithfully to community practice.</p><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">AS trends</span><p id="par0110" class="elsevierStylePara elsevierViewall">AS has become the standard of care for the initial management of localized PCa in a few countries (such as the United States, Sweden or the United Kingdom) in an attempt to preserve urinary and erectile function without missing the window for curative treatment.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Usage rates reported go from 21 to 42% in different settings, translating confidence and safety in this type of management. But a major concern with AS still exists, whether patients with low-risk cancer are being appropriately selected for treatment.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11–14</span></a></p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">AS criteria</span><p id="par0115" class="elsevierStylePara elsevierViewall">AS is a proven valuable strategy in low risk PCa patients if patients are carefully chosen. While there is some consensus about patient selection across the highly regarded international guidelines based mostly in large case series, no unanimity exists for the best follow-up protocols, in order to identify disease progression when it occurs as well as to be able to implement definitive treatment while it remains effective (<a class="elsevierStyleCrossRef" href="#tbl0035">Table 7</a><a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15–20</span></a>). AS is not without risk or disadvantages: there are reports of patients who were misclassified as having insignificant prostate cancer and later went on to disease progression. Patients also demonstrate increased levels of anxiety of living with untreated prostate cancer and invasive follow-up (with frequent examinations and prostate biopsies) is not at all avoidable.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> In our survey, management program reliability was not a major issue but associated psychological morbidity and availability of technical resources for periodic follow-up (MRI/Biopsy) were named as contributing factors for failure of AS management in 54.7% and 49.1% of inquiries, respectively.</p><elsevierMultimedia ident="tbl0035"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">Due to recent key advances in prostate Magnetic Resonance Imaging (MRI), this technique has become of great clinical utility in men with low-risk disease who are considering AS in order to guarantee that higher grade disease has not been unnoticed (anterior high-grade cancers which may be missed by conventional TRUS [trans-rectal ultrasound] guided biopsies) particularly if high strength magnets (as 3 T) and endorectal coils are used. Targeted and systematic biopsies need to be used in conjunction in order to increase detection rate.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> When performing a combined biopsy approach the result is an upgrade in tumor histopathology in 35–40% of cases from a low-risk disease to a significant PCa, with a likely correlation between clinically significative PCa and the PI-RADS (Prostate Imaging Reporting & Data System) score.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23–25</span></a> However, as the ProtecT study shows, MRI is not compulsory. In some countries it is considered as optional or investigational, except, for instance, in the United Kingdom where the National Institute for Clinical Excellence (NICE, 2019) guidelines consider it mandatory if an AS strategy is pursued.<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">19,26</span></a> Recent studies show increasing interest in this technique given that MRI-only based protocols may substitute for MRI + biopsies altogether.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> However, some caution must be taken with MRI given its two major limitations: (1) low specifity for PI-RADS 3<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> and (2) false-negatives, depending on the quality of the scan and its interpretation.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">29,30</span></a> And in some cases, as the survey identified, MRI (or even biopsy equipment) may not be readily available forfeiting an AS strategy from the very beginning (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). Finally, ISUP2014 re-classification made sure that all cancers harboring a cribriform pattern (responsible for a less favorable outcome) were attributed a grade 4, when in the original classification system, they could be assigned a Gleason 6 (3 + 3) score, harming prediction accuracy in selecting AS patients. AS protocol results set at the time may be prejudiced for insignificant prostate cancer.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> Given that our study population includes urologists with experience that dates back to even before that time, we may have captured some of these findings: about 21% of urologists surveyed referred that AS management programs lack reliability (tumor progression may occur unknowingly).</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Recommending AS</span><p id="par0125" class="elsevierStylePara elsevierViewall">The first and most important finding is that AS has gained broader acceptance as a management strategy for low-risk PCa among Portuguese urologists: close to 80% of urologists discuss AS and recommend it to their patients. They feel that it is safe and that there isn’t any difference in overall survival by waiting for the right timing for curative treatment. These findings are close to ones found in the literature in which over 80% observance rates were also found, with urologists recommending it comfortably.</p><p id="par0130" class="elsevierStylePara elsevierViewall">We also sought to elucidate the perceived attitudes regarding clinical factors when choosing an AS strategy (age, ISUP grade, T staging, PSA serum level, number of positive cores in biopsy, percentage of core involvement). When looking at clinical factors as optimal for patient selection, the overwhelming majority consider the following: grade group 1 disease, a low PSA serum level (≤10 ng/mL) and age. However, for local disease (T) as well as for the number of positive cores in prostate biopsy, half the respondents go as far as stating that localized disease is of relative importance considering that patients with disease >T2a may also be subject to AS. This relates to the AS protocols published to date, and that are transitioning to including ever-more patients in the intermediate-risk segment.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Looking closer at how these criteria fare according to years of experience, a sub-analysis of clinicians under 10 years of experience vs over/or 10 years of experience shows that only 34% of the latter find that local disease is important against 67% of the former, with younger and less experienced clinicians find local disease to be an important criterion. Almost the same proportion is seen in the criteria “percentage of core involvement”. As for the other results, they are indistinguishable in relation with experience (<a class="elsevierStyleCrossRefs" href="#tbl0025">Tables 5 and 6</a>). When comparing for statistical significance, only the criteria “workplace setting” (p = 0.047) stands out. As it seems, urologists at university and community hospitals favor AS more than their peers in private practice (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 6</a>). We found it hard to compare to other workplace settings as most foreign case series are from single center high-volume university/community hospitals or all the three different settings combined. The former, in methodology, identify that bias whereas the latter, although translating a real-word representation, fail to discriminate the population according to workplace setting of inclusion. Also, in Portugal there are no private university hospitals which may not the case elsewhere.</p><p id="par0140" class="elsevierStylePara elsevierViewall">We found it interesting that while AS is undisputed as a solid management strategy for the respondents, there is a general belief that their low-risk patients were not interested in AS and rather chose to undergo immediate treatment (52% forfeit AS). While attitudes towards usage of current surveillance methods were generally positive, there are some concerns about employing AS universally. One may argue that not all low-risk patients that fit the criteria listed previously are ideal for AS, particularly patients with high-volume disease, family history of early mortality from PCa, and significant anxiety associated with the monitoring process.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Intermediate-risk patients</span><p id="par0145" class="elsevierStylePara elsevierViewall">To extend AS for intermediate-risk patients is still controversial despite the ProtecT trial providing Level 1 evidentiary support.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> AS is feasible for selected men, however it is difficult to accurately measure disease risk and act accordingly, particularly in patients harboring occult high-grade histopathologic features. Survival rates between intermediate- and low-risk patients do not differ at 5 years, but are worst at 10 years. Advanced imaging and novel and reliable biomarker technology are still needed to consider these patients up for an individualized AS program.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">31,32</span></a></p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Incorporating MRI and biomarkers in active surveillance protocols</span><p id="par0150" class="elsevierStylePara elsevierViewall">MRI and biomarkers can enhance two aspects of AS: patient selection and monitoring patients afterwards. Frequent serial biopsies contribute to patient burden, side effects and fallout of follow-up, particularly the trans-rectal biopsy. Non-invasive methods that can achieve balance between overtreatment and under-diagnosis are required. As of today, with the current level of evidence, it is not possible to supplant prostate biopsies with MRI in AS protocols.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">33,34</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">The most commonly used serum biomarker to detect and monitor PCa is PSA, however, PSA-only monitoring is insufficient: many novel genetic and epigenetic markers of PCa disease status are under investigation. Biomarkers being validated in AS protocols, that are currently under investigation for prostate cancer can be seric, urinary or tissue-related. Future directions indicate that in a first stage these will be combined with imaging findings (MRI) and later on be individualized, accounting for patient variables, predicting a true pathologic state and eliminating the need for biopsy or even MRI in men with low risk PC altogether.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">35,36</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">Decisions concerning treatment options frequently depend not only on patient and provider preferences but also on resource availability. Lack of adoption may be because of the advent of less invasive forms of prostatectomy (i.e., robotic surgery). It would be interesting to ask urologists if access to robot-assisted surgery (or other forms of minimally invasive surgery), or access to multidisciplinary clinic would change the pattern of answers, given that treatment choice often reflects the specialist recommendation over patient preference and values. Also, the availability of other forms of less invasive radiotherapy treatments (e.g. brachytherapy) may be relevant. The rise in use of surgical or novel radiotherapy likely reflects diverse factors that cannot be captured in this study, though this is a ripe area of study to determine factors influencing treatment decisions for these patients.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Adherence to AS protocol</span><p id="par0165" class="elsevierStylePara elsevierViewall">A barrier to AS remains the burden of morbidity associated with prostate biopsies: the need for a repeat prostate needle biopsy on the same patient may pay a hefty price since the rate of biopsy related infectious complications also increases.<a class="elsevierStyleCrossRefs" href="#bib0205"><span class="elsevierStyleSup">37,38</span></a> There may be an increase in patient costs associated with serial office visits and the burden on physicians and healthcare systems stemming from the increased clinical volume following these patients. Generally, patients on AS show control over their cancer and comfort with the AS protocol over time and experience slight decreases in anxious preoccupation, cancer-related avoidance thoughts and behaviors, and fight-against-cancer reactions, however some suggest men should follow a longitudinal psychological periodic assessment to promptly detect changes in coping strategies needing intervention.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">36</span></a> One of the major findings of the CaPSURE registry<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> was that all of the participating urologists (most of whom were community-based) treat patients according to their usual practice preferences, not following any specified protocols or guidelines. In a similar fashion, a balanced nationwide survey, covering both academic, private and community practice has that advantage of real-life representation. Thus, this database offers a good starting point for discussion and reflection of contemporary urology practice at the national level as is available.</p><p id="par0170" class="elsevierStylePara elsevierViewall">There are some limitations of our study that warrant discussion:<ul class="elsevierStyleList" id="lis0015"><li class="elsevierStyleListItem" id="lsti0035"><span class="elsevierStyleLabel">-</span><p id="par0175" class="elsevierStylePara elsevierViewall">Small sample size due to a 15% response rate from surveyed urologists;</p></li><li class="elsevierStyleListItem" id="lsti0040"><span class="elsevierStyleLabel">-</span><p id="par0180" class="elsevierStylePara elsevierViewall">Furthermore, because responses were in the format of multiple choice, the full range of opinions might not have been adequately captured;</p></li><li class="elsevierStyleListItem" id="lsti0045"><span class="elsevierStyleLabel">-</span><p id="par0185" class="elsevierStylePara elsevierViewall">Being an online inquiry, the sample is not truly accountable statistic representation of the urology community in Portugal. An auto-selection bias and availability to answer is in place.</p></li><li class="elsevierStyleListItem" id="lsti0050"><span class="elsevierStyleLabel">-</span><p id="par0190" class="elsevierStylePara elsevierViewall">Acquiescence bias or social desirability bias – in which participants of the survey choose the options they understand as the standard of care, recommended by national and international guidelines and are not the ones they use in clinical practice, due to a culture of institutionalization that encourages and incentivizes eagerness to please and to try to appear more favorable to the examiner than their true belief;</p></li><li class="elsevierStyleListItem" id="lsti0055"><span class="elsevierStyleLabel">-</span><p id="par0195" class="elsevierStylePara elsevierViewall">Survey fatigue – can result in responses that are not genuine; there was no financial incentive to complete the survey, we hope answers are legitimate;</p></li><li class="elsevierStyleListItem" id="lsti0060"><span class="elsevierStyleLabel">-</span><p id="par0200" class="elsevierStylePara elsevierViewall">The small sample didn’t allow us to consider subgroups of community vs academic practice and analyze answers accordingly. This is important as community practice may translate a context where urologists and patients don’t have the technical requirements to manage low risk patients safely (availability of MRI, 3 T, endorectal coil);</p></li><li class="elsevierStyleListItem" id="lsti0065"><span class="elsevierStyleLabel">-</span><p id="par0205" class="elsevierStylePara elsevierViewall">We take for granted that urologists practicing in Portugal follow EAU guidelines but that may be biased as some may rather consider other their institution, national or international recommendations, and probably this would be a subject of another survey question in the future;</p></li><li class="elsevierStyleListItem" id="lsti0070"><span class="elsevierStyleLabel">-</span><p id="par0210" class="elsevierStylePara elsevierViewall">We didn’t account for race when choosing patient selection (another bias carried in the literature in landmark studies as the ProtecT trial<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> nor histopathologic findings like intraductal, ductal, and invasive cribriform types already featured in the NCCN guidelines<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>;</p></li><li class="elsevierStyleListItem" id="lsti0075"><span class="elsevierStyleLabel">-</span><p id="par0215" class="elsevierStylePara elsevierViewall">We did not distinguish respondents who were primarily trained in general urology or urologic oncology as the reality for Portugal is that most urologists practice general urology. Separating high-volume urologists from low-volume ones (through the number of PCa cancer patients observed monthly) accounts in part for this bias, but may not be sufficient. One can still have general urology practices almost entirely dedicated to outpatient consults.</p></li></ul></p><p id="par0220" class="elsevierStylePara elsevierViewall">On a final note, should low-risk disease managed with AS be a reason to talk about systematic PCa screening? Controversies still exist whether the benefits of PSA-based screening outweigh potential harms. Screening for PCa may reduce risk of PCa mortality but is associated with harms including false-positive results, biopsy complications, and overdiagnosis. With MRI improving risk stratification and disease reclassification, it is believed overtreatment can be reduced to some extent with AS, meaning only that positive screening results don’t necessary mean active treatment. If the clinicians are proactive in offering AS, the majority of patients are ready to accept.</p></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Conclusion</span><p id="par0225" class="elsevierStylePara elsevierViewall">The intention of AS is to preserve QOL by delaying or avoiding the adverse effects of immediate treatment while monitoring for disease progression. PCa mortality increases with longer follow-up duration, but results remain acceptable, with deaths from other causes outnumbering PCa deaths. AS seems to be sustained in urologist clinical practice, although patients still lag to adhere and choose for active treatment. AS may not be an easy choice for patients and clinicians due to uncertainty of disease progression, risk of loss to follow-up and repeated biopsies but also may be a cause for anxiety, depression, uncertainty and a perception of danger. Active surveillance (AS) is a form of conservative management aimed to reduce over-treatment for low-risk PCa patients. Over the years, experience with AS has grown considerably and is now standard in some countries, however a universal protocol still does not exist.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methods</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Nationwide anonymous e-survey concerning habits and practices on AS among Portuguese urologists, that consisted of twelve questions and was sent electronically to all 368 current members of the Portuguese Urological Association.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0080" class="elsevierStyleSimplePara elsevierViewall">56 urologists were surveyed (15.21% answer rate), evenly distributed geographically and allocated according to years of experience as well as number of PCa patients managed monthly. The vast majority of respondents recommends AS to their patients, particularly ISUP grade 1 patients, whose PSA serum level is bellow 20 ng/mL. Observance of AS programs by patients was not in question but concerns exist over psychological morbidity while harboring disease. Majority believed that international guidelines surveillance protocols were adequate and sufficient, but there are some constraints concerning availability of periodic MRIs and re-biopsy needs.</p></span> <span id="abst0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Conclusions</span><p id="spar0085" class="elsevierStyleSimplePara elsevierViewall">AS seems to be sustained in urologist clinical practice, although patients still lag to adhere and choose for active treatment. La vigilancia activa (VA) es una forma de tratamiento conservador, empleado con el objetivo de limitar el sobretratamiento de los pacientes con CaP de bajo riesgo. A lo largo de los años, la experiencia con VA ha ido aumentado considerablemente, siendo actualmente el manejo estándar en algunos países. Sin embargo, no existe, a día de hoy, un protocolo universal.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Métodos</span><p id="spar0100" class="elsevierStyleSimplePara elsevierViewall">Encuesta electrónica anónima de ámbito nacional, sobre los hábitos y prácticas en materia de VA entre urólogos portugueses. La encuesta constaba de doce preguntas, y fue enviada por vía electrónica a los 368 miembros actuales de la Asociación Portuguesa de Urología.</p></span> <span id="abst0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Resultados</span><p id="spar0105" class="elsevierStyleSimplePara elsevierViewall">Se encuestó a 56 urólogos (15,21% de respuestas), distribuidos geográficamente de manera uniforme, y categorizados según los años de experiencia y el número de pacientes con CaP tratados mensualmente. La gran mayoría de los encuestados recomienda la VA a sus pacientes, en particular en el caso de pacientes de grado 1 de la ISUP, con un nivel sérico de PSA es inferior a 20 ng/mL. No se cuestiona el cumplimiento de los programas de VA por parte de los pacientes, pero la morbilidad psicológica de albergar la enfermedad, es un factor preocupante. Según la mayoría de los encuestados, los protocolos de vigilancia propuestos por las guías internacionales son adecuados y suficientes, pero se ponen de manifiesto algunas limitaciones en cuanto a la disponibilidad de la resonancia magnética de forma periódica y al requerimiento de nuevas biopsias.</p></span> <span id="abst0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Conclusiones</span><p id="spar0110" class="elsevierStyleSimplePara elsevierViewall">Aunque la VA parece mantener un lugar en la práctica clínica urológica, los pacientes siguen siendo reticentes a la hora de elegir el tratamiento, optando en muchas ocasiones por el tratamiento activo.
