This study aimed to evaluate the efficacy of hyaluronic acid gel as an adjunct to temporalis fascia graft tympanoplasty in total tympanic membrane perforation.
MethodsA prospective single blinded randomized controlled trial carried out among 60 patients with total tympanic membrane perforation randomized into 2 groups A&B each comprised of 30 patients. Patients in group A underwent underlay tympanoplasty using temporalis fascia graft through postauricular approach with the utilization of hyaluronic acid gel as an adjunct to temporalis fascia graft. Patients in group B underwent classic underlay tympanoplasty using temporalis fascia graft through postauricular approach. Both groups were compared as regards graft success rate as well as hearing outcome.
ResultsThe difference in the graft success rate between group A (93.33%) and group B (80%) was statistically significant (P value = .04). Preoperative air conduction (AC) threshold and air bong gap (ABG) showed no statistically significant differences between both groups. Postoperative AC thresholds for groups A and B were 23.55 ± 3.69 dB and 26.40 ± 5.26 dB respectively with statistically significant difference (P value = .04). Postoperative ABG for groups A and B were 8.90 ± 4.07 dB and 12.00 ± 5.41 dB respectively and this difference was also statistically significant (P value = .03).
ConclusionUtilization of hyaluronic acid gel as an adjunct to temporalis fascia graft tympanoplasty can improve the graft success rate.
Este estudio tuvo como objetivo evaluar la eficacia del gel de ácido hialurónico como complemento a la timpanoplastia con injerto de fascia temporal en la perforación total de la membrana timpánica.
MétodosSe realizó un ensayo clínico prospectivo, aleatorizado y controlado, con ciego simple, en 60 pacientes con perforación total de la membrana timpánica, distribuidos aleatoriamente en dos grupos (A y B), cada uno compuesto por 30 pacientes. Los pacientes del grupo A se sometieron a una timpanoplastia subyacente con injerto de fascia temporal por vía retroauricular, utilizando gel de ácido hialurónico como complemento. Los pacientes del grupo B se sometieron a una timpanoplastia subyacente clásica con injerto de fascia temporal por vía retroauricular. Se compararon ambos grupos en cuanto a la tasa de éxito del injerto y la audición.
ResultadosLa diferencia en la tasa de éxito del injerto entre el grupo A (93,33%) y el grupo B (80%) fue estadísticamente significativa (P = ,04). El umbral de conducción aérea (CA) preoperatorio y la brecha aérea (ABG) no mostraron diferencias estadísticamente significativas entre ambos grupos. Los umbrales de CA posoperatorios para los grupos A y B fueron de 23,55 ± 3,69 dB y 26,40 ± 5,26 dB respectivamente, con una diferencia estadísticamente significativa (P = ,04). La ABG posoperatoria para los grupos A y B fue de 8,90 ± 4,07 dB y 12,00 ± 5,41 dB respectivamente, y esta diferencia también fue estadísticamente significativa (P = ,03).
ConclusiónLa utilización de gel de ácido hialurónico como complemento de la timpanoplastia con injerto de fascia temporal puede mejorar la tasa de éxito del injerto.
