Impact of an active hospital recruitment strategy on pediatric influenza vaccination coverage

Abstract
Introduction

Influenza represents a significant public health issue in the pediatric population. Low influenza vaccination coverage among healthy children aged 6–59 months in Spain justifies evaluating active improvement strategies.

Material and methods

Prospective descriptive non-randomized intervention study during the 2024–2025 campaign at Hospital Universitario de Cruces (CAPV), targeting healthy children aged 6–59 months with appointments in 15 hospital clinics. Weekly, one day was selected for organizational convenience; patient records were reviewed to identify candidates. Pediatric Infectious Diseases staff made telephone calls providing information on influenza vaccination and offering administration on the same day and location as the scheduled consultation.

Results

2,677 children aged 6–59 months had hospital appointments during this period (October 1, 2024–March 31, 2025). The intervention strategy was applied to 651 children (24.3% of total). Families of 347 unvaccinated children were contacted by phone; 223 (64.3%) accepted and received vaccination coinciding with their hospital visit. At campaign end, 81% of the 651 study children were vaccinated (double the target population coverage in CAPV: 40.8%).

Conclusions

A simple, low-cost strategy based on active telephone outreach coordinated with hospital appointments successfully increases influenza vaccination coverage in the target population. Active professional recommendation and improved accessibility should be structurally integrated into future influenza campaigns.

Introducción

La gripe constituye un importante problema de salud pública en la edad pediátrica. La baja cobertura de vacunación antigripal en los menores sanos de 6 a 59 meses en España justifica la evaluación de estrategias activas de mejora.

Material y métodos

Estudio descriptivo prospectivo de intervención no aleatorizada durante la campaña 2024–2025 en el Hospital Universitario de Cruces (CAPV), dirigido a menores sanos de 6 a 59 meses con cita en 15 consultas hospitalarias. Semanalmente, se seleccionó un día por conveniencia organizativa y se revisaron las historias de los pacientes citados, identificando los candidatos. El personal sanitario de infectología pediátrica llamó informando sobre la vacunación antigripal, ofreciendo administrar la vacuna el mismo día y lugar de la consulta preestablecida.

Resultados

2.677 menores entre 6 y 59 meses tuvieron cita en consultas hospitalarias en este periodo de tiempo (01 de octubre de 2024 al 31 de marzo de 2025). En 651 niños (24,3% del total) se aplicó la estrategia de intervención. Se contactó telefónicamente con las familias de los 347 menores no vacunados; 223 (64,3%) aceptaron y recibieron la vacunación coincidiendo con su consulta hospitalaria. Al finalizar la campaña, el 81% de los 651 niños incluidos en el estudio estaban vacunados (el doble de la cobertura observada en la población diana de la CAPV [40,8%]).

Conclusiones

Una estrategia sencilla y de bajo coste basada en la captación activa telefónica, coordinada con las citas hospitalarias, consigue aumentar la cobertura vacunal antigripal en la población diana. La recomendación activa del profesional y la mejora de la accesibilidad deberían integrarse de forma estructurada en futuras campañas antigripales.
