Introdução

A disfunção temporomandibular (DTM) é um distúrbio que afecta as articulações tempomandibulares e a musculatura da mastigação do aparelho estomatognático e as estruturas associadas. O espasmo dos músculos da mastigação é o principal responsável pela sintomatologia dolorosa na DTM e este pode ser causado pela presença de hábitos parafuncionais, os quais podem ser desencadeados ou agravados pelo stresse emocional. Além dos factores psicológicos, a presença de alterações na coluna cervical pode provocar modificações no posicionamento da mandíbula, alterando toda a estrutura musculo-ligamentar da articulação temporomandibular (ATM), levando ao desenvolvimento da DTM. Estudos nacionais sobre a prevalência de DTM e a sua relação com alterações posturais e com os níveis de ansiedade e depressão são desconhecidos.

Objectivos

Verificar a prevalência de DTM e a sua associação com factores psicológicos, nomeadamente ansiedade e depressão, e alterações na coluna cervical em alunos da Escola Superior de Saúde Jean Piaget do Algarve (ESSJPA).

Material e métodos

A amostra foi constituída por 306 alunos dos cursos de Licenciatura em Fisioterapia, Enfermagem, Farmácia e Análises Clínicas e de Saúde Pública, de ambos os géneros e com idades compreendidas entre os 18 e os 43 anos. Os instrumentos de medida foram aplicados em apenas um momento, durante o ano lectivo 2009/2010, sendo utilizados o Questionário Anamnésico de Fonseca para avaliar a presença de DTM, a Escala de Medida de Ansiedade e Depressão (HAD) e realizou-se a avaliação postural da coluna cervical.

Resultados e discussão

Os resultados obtidos indicaram uma prevalência de DTM em 37,3% dos indivíduos da amostra e 18,3% revelaram sinais de ansiedade ou depressão; quanto à avaliação postural, verificou-se que 36,6% dos alunos apresentaram rectificação da curvatura cervical e 19,6% hiperlordose cervical. Ao relacionar a presença de DTM com as alterações na coluna cervical, verificou-se que não existiu relação estatisticamente significativa (p>0,05). No entanto, a associação entre o DTM com os níveis de ansiedade e depressão, revelou que dos 114 alunos (37,3%) que apresentavam sintomatologia de DTM, 35 (11,4%) também apresentaram sinais de ansiedade ou depressão, sendo esta associação estatisticamente significativa (p<0,05).

Conclusão

O presente estudo verificou uma elevada prevalência de DTM em alunos da ESSJPA de diversos cursos na área da saúde, sendo constatado uma relação entre esta disfunção com os níveis de ansiedade ou depressão. A divulgação do tema junto de populações sujeitas a grande stresse emocional, como são os estudantes do ensino superior, revela-se de grande importância, para que os próprios estejam mais atentos aos sinais e sintomas de DTM.

Introduction

The temporomandibular disorder (TMD) affects the tempomandibular joints and stomatognathic apparatus mastication muscles and associated structures. The mastication muscles spasm is the main cause of painful symptoms in TMD; this situation may be induced by deleterious habits, which can be triggered or aggravated by emotional stress. In addition to psychological factors, cervical spine postural alterations may lead to jaw repositioning and structural changes in the whole temporomandibular joint (TMJ), causing TMD. National studies on the TMD prevalence and its relation with postural changes and levels of anxiety and depression are unknown.

Objective

The objective of this study was to investigate the prevalence of TMD and its association with psychological factors, particularly anxiety and depression, and also postural changes in the cervical spine, in students from the Jean Piaget-Algarve Health School.

Material and methods

The sample comprised 306 students of Physiotherapy, Nursing, Pharmacy and Clinical Analysis and Public Health BScs, from both genders, aged 18 to 43 years. TMD (using Fonseca Anamnesic Questionnaire), Anxiety and Depression and cervical spine posture were evaluated once during the 2009/2010 academic year.

Results and discussion

The results indicated a TMD prevalence of 37.3% and 18.3% of anxiety or depression. The postural evaluation showed that 36.6% of students had correct cervical curvature and 19.6% had cervical concavity. No statistically significant relation (p>0.05) was found between the presence of TMD and cervical spine changes. However, the association between TMD and anxiety and depression, showed that from the 114 students (37.3%) who had TMD symptoms, 35 (11.4%) also showed anxiety or depression signs (statistically significant; p<0.05).

Conclusion

This study showed that the Jean Piaget-Algarve Health School students’, from various health courses, present a high TMD prevalence, and revealed a significant association between this dysfunction and anxiety or depression levels. It is of great importance to disclosure this subject to populations’ submitted to great emotional stress, such as university students, in order to improve individual self awareness to TMD signs and symptoms.
