Abstract

Carcinoma of the rete testis is a rare malignant tumor which frequently occurs in middle-aged to older patients and has an aggressive biological behavior. We present the case of a 57-year-old man who presented with an ill-defined mass in the right testicle. The patient underwent a radical orchidectomy. Microscopic evaluation showed a neoplasm displaying a complex papillary-cystic architecture, infiltrating the testicular parenchyma. An in situ proliferation of neoplastic cells, with nuclear stratification and scanty cytoplasm was seen at the periphery, within the channels of the rete testis. The tumor infiltrated the tunica albuginea focally without disrupting it completely. Immunohistochemistry was positive for AE1/AE3, CK7, CK34βE12, D2-40, and PAX8. Imaging studies presented no evidence of metastatic disease. These findings are those of a primary rete testis carcinoma. The transition between benign and neoplastic rete testis epithelium served as a helpful diagnostic clue. Metastatic carcinomas from other sites were considered in the differential diagnosis.

El carcinoma de la rete testis es un tumor maligno inusual, que ocurre con frecuencia en pacientes de mediana edad a mayores y tiene un comportamiento biológico agresivo. Presentamos el caso de un hombre de 57 años que consultó por una masa mal definida en el testículo derecho. El paciente se sometió a una orquiectomía radical. La evaluación microscópica mostró una neoplasia maligna con una arquitectura papilar-quística compleja, infiltrando el parénquima testicular. En la periferia, había una proliferación in situ de células neoplásicas, con estratificación nuclear y escaso citoplasma, dentro de los canales de la rete testis. El tumor infiltró la túnica albugínea focalmente sin alterarla por completo. La inmunohistoquímica fue reactiva para la citoqueratina AE1/AE3, CK7, CK34βE12, D2-40 y PAX8. Los estudios de imagen no presentaron evidencia de enfermedad metastásica. Estos hallazgos fueron los de un carcinoma primario de la rete testis. La transición entre el epitelio benigno y neoplásico de la rete testis sirvió como una pista diagnóstica útil. Los carcinomas metastásicos de otros sitios se consideraron en el diagnóstico diferencial.
