Renal and Hematologic Toxicity Profile of [177] Lu-labeled PSMA-targeted radioligand-therapy (177Lu-PSMA-RLT): A Retrospective Cohort Study

Abstract

Objective

This study aims to retrospectively evaluate the renal and hematologic toxicity profile of ¹⁷⁷Lu-PSMA-RLT in patients with metastatic castration-resistant prostate cancer.

Methods

Renal function (GFR, BUN, and creatinine) and hematological parameters (hemoglobin, leukocyte, platelet) before and after treatment were examined in 41 patients who received ¹⁷⁷Lu-PSMA-RLT at our clinic, and changes along with toxicity grades were determined. Additionally, pre-treatment PSMA PET/CT findings and other demographic and clinical data were assessed.

Results

In terms of renal toxicity, G0-G1 level toxicity was observed in 97.6% of cases, while G2 toxicity was observed in only one patient. This patient already had a low GFR before treatment and experienced only a 2-unit decrease. Regarding hematological toxicity, the rates of G3–4 toxicity were as follows: leukocytes 7.3%, hemoglobin 9.8%, platelets 7.3%.

Conclusion

No clinically significant nephrotoxicity was observed with ¹⁷⁷Lu-PSMA-RLT. Hematologic toxicity is significant but manageable, and severe side effects are rare and within predictable limits.

Objetivo

Este estudio tiene como objetivo evaluar retrospectivamente el perfil de toxicidad renal y hematológica de ¹⁷⁷Lu-PSMA-RLT en pacientes con cáncer de próstata metastásico resistente a la castración.

Métodos

En 41 pacientes que recibieron ¹⁷⁷Lu-PSMA-RLT en nuestra clínica, se examinaron la función renal (TFG, BUN y creatinina) y los parámetros hematológicos (hemoglobina, leucocitos, plaquetas) antes y después del tratamiento, determinándose los cambios y los grados de toxicidad. Adicionalmente, se evaluaron los hallazgos previos al tratamiento en la PET/TC con PSMA y otros datos demográficos y clínicos.

Resultados

En cuanto a la toxicidad renal, se observó toxicidad de nivel G0-G1 en el 97,6% de los casos, mientras que solo un paciente presentó toxicidad G2. Este paciente ya tenía una TFG baja antes del tratamiento y experimentó solo un descenso de 2 unidades. Respecto a la toxicidad hematológica, las tasas de toxicidad G3–4 fueron: leucocitos 7,3%, hemoglobina 9,8%, plaquetas 7,3%.

Conclusión

No se observó nefrotoxicidad clínicamente significativa con la terapia con radioligandos ¹⁷⁷Lu-PSMA-RLT. La toxicidad hematológica, aunque significativa, es manejable, y los efectos secundarios graves son raros y se mantienen dentro de límites previsibles.
