Abstract

Submental intubation, first described in 1986, is an effective alternative to tracheostomy in patients with complex facial fractures in whom nasotracheal intubation is contraindicated and orotracheal intubation would interfere with the surgical field. We report the case of a 38-year-old man with a history of morbid obesity who suffered a panfacial fracture with involvement of the frontal sinuses and intracranial communication after falling from a second floor. Nasotracheal intubation was contraindicated, and orotracheal intubation would have interfered with the planned intermaxillary fixation. We first performed fibreoptic orotracheal intubation, which was then surgically converted to submental intubation. No intraoperative or postoperative complications occurred, and the patient’s evolution was favourable, with only minimal scarring in the submental region. Recent literature has confirmed that this technique is safe and is associated with less morbidity and better aesthetic outcomes than tracheostomy. This case highlights the need to consider submental intubation as a priority airway management option in complex panfacial fractures.

La intubación submentoniana, descrita en 1986, constituye una alternativa eficaz a la traqueostomía en pacientes con fracturas faciales complejas en quienes la intubación nasotraqueal está contraindicada y la orotraqueal interfiere con el campo quirúrgico. Se presenta el caso de un varón de 38 años, con antecedente de obesidad mórbida, que tras precipitación desde un segundo piso sufrió fractura panfacial con compromiso de senos frontales y comunicación intracraneal. La intubación nasotraqueal estaba contraindicada, y la fijación intermaxilar planificada impedía la vía orotraqueal. Se realizó intubación orotraqueal con fibroscopio flexible y, posteriormente, conversión a intubación submentoniana mediante abordaje quirúrgico, sin complicaciones intraoperatorias ni postoperatorias. La evolución clínica fue favorable, con cicatriz estética y mínima en la región submentoniana. La literatura reciente confirma que esta técnica es segura, con menor morbilidad y mejor resultado estético que la traqueostomía. Este caso destaca la necesidad de considerar la intubación submentoniana como opción prioritaria en el manejo de la vía aérea en fracturas panfaciales complejas.
