Abstract
Background

Lumbar plexus block (LPB) can be used with different techniques for anaesthesia and postoperative analgesia during femoral surgery. This study aimed to compare in-plane and out-of-plane methods using the Shamrock technique.

Methods

Forty-two patients scheduled for surgery due to femoral neck fractures were included in this randomized controlled study. The Shamrock technique was used to visualise the lumbar plexus. Patients were randomized to 2 groups: one group received LPB using the in-plane technique (IP group, n = 20), and the other received LPB using the out-of-plane technique (OOP group, n = 20). The primary outcome measure was block procedure time. Secondary outcomes included the number of needle insertions and redirections, needle depth (cm), minimal nerve stimulation (mA), sensory and motor block levels, and epidural spread.

Results

No statistically significant difference was found between the groups in block procedure times (p > 0.05).The number of insertions and the number of needle redirections were significantly lower in the OOP group compared to the IP group (p = 0.005). The needle depth was significantly greater in the IP group compared to the OOP group (6.5 ± 1.1 cm vs 5.1 ± 0.8 cm, p = 0.001). No difference was found in sensory and motor block times (p > 0.05). No differences were observed in the number of patients with epidural spread or in the incidence of complications (p > 0.05).

Conclusion

The OOP technique demonstrated similar efficacy to the IP method using the Shamrock technique. The OOP technique can be used as an alternative for elderly patients in whom the IP technique can be difficult.

Antecedentes

El bloqueo del plexo lumbar (BPL) puede utilizarse con diferentes técnicas para anestesia y analgesia postoperatorias durante la cirugía femoral. El objetivo de este estudio fue comparar los métodos de guía de la aguja dentro y fuera del plano utilizando el abordaje tipo Shamrock.

Métodos

En este estudio controlado aleatorizado se incluyeron cuarenta y dos pacientes programados para cirugía debido a fracturas de cuello del fémur. Se utilizó abordaje tipo Shamrock para visualización del plexo lumbar. Los pacientes fueron aleatorizados en dos grupos: un grupo recibió BPL utilizando la técnica dentro del plano (n = 20), y el otro grupo recibió BPL utilizando la técnica fuera del plano (n = 20). La medida del resultado primario fue el tiempo del procedimiento de bloqueo. Los resultados secundarios incluyeron el número de inserciones y redirecciones de la aguja, la profundidad de la aguja (cm), la estimulación mínima nerviosa (mA), los niveles de bloqueo sensorial y motor, y el grado de dispersión epidural.

Resultados

No se encontró diferencia estadísticamente significativa entre los grupos en cuanto a tiempos del procedimiento de bloqueo (p > 0,05). El número de inserciones y el número de redirecciones de la aguja fueron significativamente menores en el grupo OOP, en comparación con el grupo IP (p = 0,005). La profundidad de la aguja fue significativamente mayor en el grupo dentro del plano, en comparación con el grupo fuera del plano (6,5 ± 1,1 cm vs 5,1 ± 0,8 cm, p = 0,001). No se encontró diferencia en cuanto a tiempos de bloqueo sensorial y motor (p > 0,05). No existió diferencia en cuanto al número de pacientes con dispersión epidural ni de complicaciones observadas (p > 0,05).

Conclusión

La técnica fuera del plano demostró una eficacia similar a la del método dentro del plano en el abordaje tipo Shamrock. La técnica fuera del plano puede utilizarse como opción alternativa para pacientes mayores con dificultad en términos de guía de la aguja.
