Investigation of dye spread following ultrasound guided injection at the femoral nerve, proximal and distal adductor canal – A cadaver study

Abstract
Background

Total knee arthroplasty (TKA) is associated with significant postoperative pain. Regional analgesic techniques targeting femoral nerve (FN) and adductor canal (AC) are commonly employed; however, uncertainty persists regarding optimal injection site within the AC. Cadaveric investigations provide anatomical insights into injectate spread patterns, but their interpretation requires caution when extrapolating to clinical practice.

Methods

Twelve thighs from six embalmed human cadavers underwent ultrasound-guided injections of different-colored dyes, with equal volumes administered at three predefined locations: FN, proximal AC, and distal AC. Subsequent anatomical dissections were performed to assess nerve staining patterns. Injection sites and volumes were standardized to allow for comparative anatomical evaluation.

Results

Femoral-level injections stained the femoral nerve and its major branches. Proximal AC injections consistently stained both the saphenous nerve and nerve to vastus medialis, whereas distal canal injections predominantly stained the saphenous nerve with only variable distal obturator involvement. The vaso-adductor membrane stained consistently after proximal canal injection. No sciatic nerve staining was observed.

Conclusion

Equal-volume dye injections at the FN, proximal AC, and distal AC demonstrate distinct patterns of neural staining. Proximal AC injections resulted in consistent staining of both SN and NVM, whereas distal injections predominantly involved SN. These findings provide anatomical relevance regarding adductor canal block techniques but do not imply clinical superiority of any technique. Clinical correlation is required before extrapolating these observations to clinical practice.

Antecedentes

La artroplastia total de rodilla (ATR) está asociada a un dolor postoperatorio significativo. Las técnicas de anestesia regional dirigidas al nervio femoral (NF) y al canal aductor (CA) son frecuentemente utilizadas; sin embargo, persiste la incertidumbre relativa al sitio de inyección óptimo dentro del CA. Los estudios en cadáveres aportan información anatómica sobre los patrones de dispersión de la sustancia inyectada, aunque su interpretación requiere cautela a la hora de extrapolarse a la práctica clínica.

Métodos

Se aplicaron inyecciones ecoguiadas con tinciones de diferentes colores a 6 muslos de cadáveres humanos embalsamados, con volúmenes iguales administrados en 3 localizaciones predefinidas: NF, CA proximal y CA distal. Se realizaron disecciones anatómicas subsiguientes para evaluar los patrones de tinción del nervio. Se estandarizaron los sitios de inyección y los volúmenes para permitir una evaluación anatómica comparativa.

Resultados

Las inyecciones a nivel femoral tiñeron el nervio femoral y sus ramas principales. Las inyecciones en el CA proximal tiñeron consistentemente tanto el nervio safeno como el nervio del músculo vasto medial, mientras que las inyecciones en el canal distal tiñeron principalmente el nervio safeno con sólo un compromiso variable del nervio obturador distal. La membrana vasto-aductora se tiñó consistentemente tras la inyección en el canal proximal. No se observó tinción del nervio ciático.

Conclusión

Las inyecciones de igual volumen de tinción en el NF, el CA proximal y el CA distal demuestran patrones distintos de tinción neural. Las inyecciones en el CA proximal se tradujeron en tinción consistente tanto del nervio safeno como del nervio del vasto medial, mientras que las inyecciones distales implicaron principalmente al nervio safeno. Estos hallazgos aportan relevancia anatómica en cuanto a las técnicas de bloqueo del canal aductor, aunque no implica superioridad clínica de ninguna de las técnicas. Se requiere una correlación clínica antes de extrapolar estas observaciones a la práctica clínica.
