Interscalene block versus upper trunk block in shoulder arthroscopy: Randomized comparative study of the ease between the two techniques among residents

Abstract
Introduction

Interscalene brachial plexus block (ISB) remains the gold standard nerve block in shoulder arthroscopic surgery. However, some anatomical variations exist in addition to several reported complications and technical challenges. Upper trunk block (UTB) may be a feasible alternative. The primary outcome was the success rate of UTB and ISB. Secondary outcomes included: duration of block performance, guidance interventions, visualization of anatomic structures, postoperative pain and opioid consumption, and diaphragmatic function.

Methods

One hundred and twenty patients were divided randomly into 2 groups; 60 patients received ISB before general anaesthesia (GA) and 60 patients received UTB before GA. All blocks were performed by anaesthesia trainees under supervision of an attending consultant.

Results

Age, sex, BMI, and duration of surgery were not different statistically. The duration of block performance was significantly longer in the ISB group (P = 0.002). Guidance interventions in the ISB group were higher than the UTB group (P < 0.001). There were no significant differences regarding the worst postoperative pain score (P = 0.574), postoperative nalbuphine consumption (P = 0.813) or success rate (P = 0.31) between groups. Visualization of the anatomic structures in the 2 groups was not statistically significant (P = 0.183). Diaphragmatic function was significantly affected in the ISB group (45% complete paralysis and 35% paresis) versus (8.3% complete paralysis and 30% paresis) in the UTB group.

Conclusions

UTB may be considered a safer and a technically easier approach than ISB for providing equivalent intra and postoperative regional analgesia in arthroscopic shoulder surgery by anaesthesia trainees.

Introducción

El bloqueo interescalénico del plexo braquial (BIS) sigue siendo el bloqueo nervioso de referencia en la cirugía artroscópica de hombro. Sin embargo, existen diversas variaciones anatómicas además de diversas complicaciones y dificultades técnicas reportadas. El bloqueo del tronco superior (BTS) puede ser una alternativa factible. El resultado primario fue la tasa de éxito de BTS y BIS. Los resultados secundarios incluyeron: duración de la ejecución del bloqueo, intervenciones guiadas, visualización de las estructuras anatómicas, dolor postoperatorio y consumo de opioides, y función diafragmática.

Métodos

Se dividieron aleatoriamente ciento veinte pacientes en dos grupos; 60 pacientes recibieron BIS y 60 pacientes recibieron BTS antes de la anestesia general (AG). Todos los bloqueos fueron realizados por residentes de anestesia bajo la supervisión de un especialista tratante.

Resultados

La edad, sexo, IMC y duración de la cirugía no fueron estadísticamente diferentes. La duración de la ejecución del bloqueo fue significativamente mayor en el Grupo BIS (P = 0,002). Las intervenciones de orientación en el Grupo BIS fueron más numerosas que en el Grupo BTS (P < 0,001). No se produjeron diferencias significativas con respecto a la puntuación de peor dolor postoperatorio (P = 0,574), consumo postoperatorio de nalbufina (P = 0,813) o tasa de éxito (P = 0,31) entre los grupos. La visualización de las estructuras anatómicas en los dos grupos no fue estadísticamente significativa (P = 0,183). La función diafragmática se vio significativamente afectada en el Grupo BIS (45% de parálisis completa y 35% de paresia) frente a (8,3% de parálisis completa y 30% de paresia) en el Grupo BTS.

Conclusiones

BTS puede ser considerada una técnica más segura y técnicamente más fácil que ISB para el aporte de analgesia regional intraoperatoria y postoperatoria equivalente en la cirugía artroscópica de hombro realizada por residentes de anestesia.
