Abstract

Glucagon-like peptide type 1 analogues (aGLP1) are a group of drugs capable of stimulating the GLP1 receptor, a key peptide in glycaemic homeostasis through its actions at the pancreatic and gastric level.

This narrative review summarises the possible implications of the perioperative use of this group of drugs. The pancreatic and extrapancreatic effects of aGLP1s determine their anaesthetic implications and perioperative management. The repercussions of their long-term suspension and their effects on the immune system have not yet been sufficiently clarified. The perioperative use of GLP1 agonists is associated with better glycaemic control without a significant increase in complications. However, their use involves a delay in gastric emptying dependent on multiple factors, which may increase the risk of bronchoaspiration.

At present, we can conclude that the perioperative maintenance of GLP1 agonists optimises glycaemic control and avoids disruptions to the potential benefits of treatment without demonstrating an increase in postoperative complications. Delayed gastric emptying and possible bronchoaspiration require measures to minimise risk and ensure patient safety.

Los análogos del péptido similar al glucagón tipo 1 (aGLP1) son un grupo de fármacos capaces de estimular el receptor del GLP1, péptido fundamental en la homeostasis glucémica por sus acciones pancreáticas y gástricas. En esta revisión narrativa se resumen las posibles implicaciones del uso perioperatorio de este grupo de fármacos.

Los efectos pancreáticos y extrapancreáticos de los aGLP1 condicionan las implicaciones anestésicas y su manejo perioperatorio. La repercusión de su suspensión a largo plazo y los efectos sobre el sistema inmunitario no han sido suficientemente aclarados hasta el momento. El uso perioperatorio de aGLP1 se relaciona con un mejor control glucémico sin aumento significativo de las complicaciones, sin embargo, su empleo implica un retraso del vaciamiento gástrico dependiente de múltiples factores, lo que incrementa el riesgo de broncoaspiración.

En el momento actual podemos concluir que el mantenimiento perioperatorio de los aGLP1 aporta la optimización del control glucémico y evita disrupciones en el potencial beneficio del tratamiento sin demostrar un aumento en las complicaciones postoperatorias. El retraso del vaciamiento gástrico y la eventual broncoaspiración condicionan la necesidad de tomar medidas para minimizar el riesgo y garantizar la seguridad del paciente.
