Abstract

Tracheoesophageal fistula (TEF) is a life-threatening complication that may develop following prolonged intubation. Consequently, the importance of early diagnosis and intervention cannot be overstated. In the presence of TEF, orotracheal intubation (OTI) must be performed with the utmost caution. In patients with post-intubation respiratory failure or gastric distension, it is important to consider the risk of the endotracheal tube (ETT) penetrating the esophagus through the TEF.

We describe the case of a 57-year-old female patient with a history of prolonged intubation. The ETT entered the esophagus through the TEF and abutted an esophageal stent that had been placed after the TEF diagnosis, resulting in respiratory failure. The patient improved following endoscopic evaluation and intervention.

This case illustrates a rare complication that can occur during intubation in patients with TEF.

La fístula traqueoesofágica (FTE) es una complicación potencialmente mortal que puede desarrollarse tras una intubación prolongada. Por tanto, la importancia del diagnóstico y la intervención tempranos no puede infravalorarse. En presencia de una FTE, debe realizarse la intubación orotraqueal (IOT) con la mayor precaución. En pacientes con insuficiencia respiratoria o distensión gástrica tras la intubación, es importante considerar el riesgo de penetración por parte del tubo endotraqueal (TET) en el esófago a través de la FTE.

Describimos el caso de una paciente de 57 años con antecedentes de intubación prolongada. Se introdujo el TET en el esófago a través de la FTE, y colindó con un stent esofágico que había sido colocado tras el diagnóstico de FTE, causando insuficiencia respiratoria. La paciente mejoró tras la evaluación endoscópica y la intervención.

Este caso ilustra una complicación rara que puede producirse durante la intubación en pacientes con FTE.
