“I’m not a good enough mom”: Exploring the motherhood experience in women with fibromyalgia syndrome (FMS)

Abstract
Introduction

Fibromyalgia syndrome (FMS) is a common condition among adult women, with its cause and treatment still largely unknown. The prevalence of this syndrome is estimated to affect 2% of the global population, with rates ranging between 61% and 90%. FMS is characterized by chronic, widespread pain in various parts of the body. Considering that women, particularly mothers, serve as the cornerstone of families, any physical or psychological distress they experience directly impacts other family members, including spouses and children. Thus, it is crucial to examine the various aspects of life for mothers diagnosed with FMS.

Objective

Accordingly, the present study sought to explore the motherhood experience in women with FMS to identify the multifaceted impacts of this condition on their lives.

Materials and methods

This qualitative study used a descriptive phenomenological approach. It examined the lived experience of motherhood among mothers with FMS who were referred to rheumatology and fibromyalgia treatment centers in Tehran in 2024. Semi-structured interviews were used to collect data. Fifteen mothers with FMS were selected and studied through purposive sampling until data saturation was reached. Data analysis was performed using Colaizzi's seven-step method.

Results

Data analysis revealed three main themes: “living with constant pain and suffering”, “fear of not being a good enough mother”, and “adaptive living with the disease”, along with six subthemes and 31 initial categories.

Conclusions

The findings demonstrate that mothers with FMS face significant challenges in caring for their children since the onset of the disease, often being unable to manage childcare and related responsibilities independently. Consequently, these mothers require psychological and social support from their spouses and families. Moreover, providing health insurance, patient education programs, family training sessions, individual and family counseling, nursing care education, and rehabilitation to enhance self-care behaviors for coping with the disease and managing symptoms is crucial for improving patients quality of life.

Introducción

El síndrome de fibromialgia (FMS) es una afección común entre las mujeres adultas, cuya causa y tratamiento aún se desconocen en gran medida. Se estima que la prevalencia de este síndrome afecta al 2% de la población mundial, con tasas que oscilan entre el 61% y el 90%. El FMS se caracteriza por un dolor crónico y generalizado en varias partes del cuerpo. Teniendo en cuenta que las mujeres, en particular las madres, son la piedra angular de las familias, cualquier sufrimiento físico o psicológico que experimenten afecta directamente a otros miembros de la familia, incluidos los cónyuges y los hijos. Por lo tanto, es fundamental examinar los diversos aspectos de la vida de las madres diagnosticadas con FMS.

Objetivo

El presente estudio buscó explorar la experiencia de la maternidad en mujeres con FMS para identificar los impactos multifacéticos de esta condición en sus vidas.

Materiales y métodos

Este estudio cualitativo utilizó un enfoque fenomenológico descriptivo. Examinó la experiencia vivida de la maternidad entre madres con FMS que fueron derivadas a centros de tratamiento de reumatología y fibromialgia en Teherán en 2024. Se utilizaron entrevistas semiestructuradas para recopilar datos. Se seleccionaron 15 madres con FMS y se estudiaron mediante un muestreo intencional hasta que se alcanzó la saturación de datos. El análisis de datos se realizó utilizando el método de siete pasos de Colaizzi.

Resultados

El análisis de datos reveló tres temas principales: “vivir con dolor y sufrimiento constantes”, “miedo a no ser una madre lo suficientemente buena” y “vida adaptativa con la enfermedad”, junto con seis subtemas y 31 categorías iniciales.

Conclusiones

Los hallazgos demuestran que las madres con FMS enfrentan desafíos significativos en el cuidado de sus hijos desde el inicio de la enfermedad, a menudo siendo incapaces de manejar el cuidado de los niños y las responsabilidades relacionadas de forma independiente. En consecuencia, estas madres requieren apoyo psicológico y social de sus cónyuges y familias. Además, brindar seguro médico, programas de educación para pacientes, sesiones de capacitación familiar, asesoramiento individual y familiar, educación en cuidados de enfermería y rehabilitación para mejorar las conductas de autocuidado para enfrentar la enfermedad y controlar los síntomas es crucial para mejorar la calidad de vida de las pacientes.
