Because it is complex and multifactorial, this phenomenon has a different prevalence depending on age, sex, ethnicity, region, and period it occurs.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">2,3</span></a> Adolescents need special attention, as it is the phase most prone to suicide, a second leading cause of death among individuals between 10 and 24 years of age.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">3–5</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The World Health Organization (WHO) considers an individual between 10 and 19 years old to be an adolescent.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a> However, authors suggest that the use of the expanded definition of adolescence (10–24 years old) would be more in line with contemporary patterns of adolescent growth, and popular understandings of this stage of life.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">6</span></a> Available evidence indicates that this is a period of several physiological, psychological, and social changes, all of which are peculiar to this age group, which increase the risk of suicide by nine times.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">7</span></a> Suicidal ideation and suicide attempt not only put adolescents’ lives in danger but also result in traumatic experiences and exacerbate psychosomatic disorders.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">5</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Regarding gender, suicide attempts in adolescence are three to nine times more common in girls, while suicide rates are two to four times higher among boys, revealing a higher number of deaths in males. Furthermore, females make the attempt at an earlier age.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">8,9</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Concerning toxicological events, according to data analyzed from the Notifiable Diseases Information System (Sinan), it is noted that 29% of cases of intoxication by suicide attempt in 2021 occurred in adolescents between 10 and 19 years old.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">10</span></a> Among the agents used, pesticides and medicines stand out as the principal causes of death, respectively. Other methods used for self-extermination, usually easily accessible by adolescents, include rodenticides, drugs of abuse, household and veterinary products, cosmetics, plants, metals, and industrial chemicals.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">8,10</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Despite several studies based on neurobiological analyses, the ability to accurately predict suicidal behaviors and death by suicide remains extremely limited.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">3</span></a> As with most other psychological phenomena, there are no universally accepted concepts of singular forms of suicidal thoughts and behaviors, given that no single factor is sufficient to elucidate the reason why a person attempts to take his own life.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">9,11</span></a> Therefore, the sociodemographic and clinical characteristics of adolescents with suicidal behavior remain controversial,<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">7,12</span></a> prompting further investigations and regional assessments for a better understanding of this phenomenon to act in the prevention of suicide and enable more appropriate interventions in individuals at risk.</p><p id="par0030" class="elsevierStylePara elsevierViewall">A quarter of teens who attempt suicide make another attempt within a year of the first attempt,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">7</span></a> and death by self-extermination is preventable, with adolescence representing an opportunity to prevent poisoning, which allows for several years of potentially saved lives. In this sense, by describing toxicological events in adolescents due to suicide attempts, it becomes possible to understand the epidemiological aspects of poisoning, intervene in this age group at an early stage, and, consequently, contribute to the reversal of this dramatic scenario for global public health. Thus, the objective of this study was to analyze factors associated with more serious outcomes of suicide attempts due to toxicological events in adolescents assisted by a Center for Information and Toxicological Assistance.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0035" class="elsevierStylePara elsevierViewall">A cross-sectional study was conducted utilizing information from the Center for Information and Toxicological Assistance of Londrina (CIATox-Londrina) of the University Hospital of the State University of Londrina (HU-UEL). High-complexity reference for the city of Londrina-PR and the surrounding area, HU-UEL is a large-scale tertiary hospital service.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In the HU-UEL emergency room, CIATox-Londrina provides intoxicated patients with clinical and laboratory care, technical advice, and advice. The service operates uninterruptedly, via telephone or in person, assisting about 100 municipalities in the northern region of the state of Paraná, Brazil, in addition to other areas of the state of Paraná and other states in Brazil.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Adolescents between 10 and 19 years old,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">1</span></a> both sexes, attended by CIATox-Londrina due to attempted suicide between 2017 and 2020, were included in the study.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Data were collected from the Brazilian Poisoning Data System (DATATOX), a system for recording, monitoring, and retrieving clinical toxicology data maintained by the Brazilian Association of Toxicological Information and Assistance Centers (ABRACIT), used by CIATox-Londrina. The purpose of DATATOX is to assist professionals from information and toxicological assistance centers, as well as allow the use of their data for epidemiological studies.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The variables analyzed for demographic characterization were sex (male; female), age (in years), age group (pre-teens – 10–14 years; adolescents – 15–19 years), pregnant women (yes; no), and ethnicity (white; brown/black/yellow).<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">13</span></a> Regarding suicide attempts, the following were evaluated: toxic agents (medicines; drugs of abuse; household or industrial household/chemical products; rodenticides; pesticides; veterinary products; cosmetics and personal hygiene products; food; household insecticides; plants/fungi; others), place of exposure (home; school; others), route of exposure (only oral; others), number of agents (one agent; two or three agents; four or more agents) and time until service (in minutes and categorized by quartile). In addition, it was evaluated the place of exposure (residence; school; others) and period of exposure (day period and weekday and month).</p><p id="par0060" class="elsevierStylePara elsevierViewall">The other variables analyzed were: clinical manifestations (yes; no), categories of signs and symptoms (neuro/psychic/muscular; cardiovascular; digestive; respiratory, among others), motivation for the suicide attempt (depression; family problems; relationship problems; anxiety/anger crisis; death of a close family member; personal problems; bad feelings; sexual abuse; others; unidentified), hospital stay (yes; no), length of hospital stay (in days), and final outcome (asymptomatic; mild; moderate; severe; fatal).<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">14</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Categorial variables were expressed as an absolute number (<span class="elsevierStyleItalic">N</span>) and a percentage (%), while continuous variables were expressed as a mean and standard deviation (SD) or median and interquartile range (IQR). For association analyses, logistic regression was used with entry data, and a hierarchic model (backward stepwise), with the calculation of the odds ratio (OR) and 95% confidence interval (95% CI) and also performed Akaike information criterion (AIC) and Bayesian information criterion (BIC) to assess the fit model. The quality of the models was compared by using the comparison criteria of AIC and BIC. The best model is the one that has the lowest values of AIC and BIC. Variance inflation factor (VIF) was used to analyze the multicollinearity between variables in the model.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The dependent variable was the final outcome, divided into two categories: asymptomatic or mild and moderate, severe or fatal.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">14</span></a> Sex, age, ethnicity, number of agents, period of exposure (day period, weekday, and month), the time to service, and exposure year were the independent variables. Statistical analyzes were performed using IBM SPSS, version 22 (IBM, Armonk, NY, USA) and R, version 3.6.2. The tests were two-tailed, and <span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05 indicated statistically significant results.</p><p id="par0075" class="elsevierStylePara elsevierViewall">This work is part of a larger project, entitled “Analysis of the activities developed and cases attended by a toxicological information center” and approved by the Research Ethics Committee of the State University of Londrina (CAAE 45986415.1.0000.5231).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0080" class="elsevierStylePara elsevierViewall">From 2017 to 2020, 1462 adolescent patients with suicide attempts were attended by CIATox-Londrina and included in this study. Of these, 17 (1.1%) were lost, as there was no information on the final outcome of the case. Therefore, 1445 cases were analyzed, whose characteristics are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. The predominant age group of adolescents was those between 15 and 19 years old (75.8%), with a median age of 16 years (IQR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>15–18). Suicide attempts were more frequent in females (79.9%) and in white/yellow individuals (76.3%). Of the total number of female adolescents, only eight (0.7%) were pregnant. It was confirmed that the majority of suicide attempts (52.9%) used a single agent, whereas the minority (15.2%) used four or more chemicals. Nearly all the suicide attempts happened orally (99.2%) and at home (97.6%).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">The median time interval from the suicide attempt to the first call for a toxicology consultation was 120<span class="elsevierStyleHsp" style=""></span>min (IQR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>240<span class="elsevierStyleHsp" style=""></span>min). A low percentage (341 patients – 23.6%) were found to need more than 24<span class="elsevierStyleHsp" style=""></span>h of hospitalization, with a mean stay of 2.4 days (±2.8).</p><p id="par0090" class="elsevierStylePara elsevierViewall">The spring months of October and November saw the highest rates of suicide attempts (10.1% and 10.4%, respectively), with Monday having the highest frequency (17.3%). The majority of the events (36.2%) happened between 6:00 and 11:59 pm. The monthly variation of suicide attempts is presented in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. In contrast, the distribution by weekday and day period of suicide attempts is shown in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> (<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1401 cases due to cases without records of the day of the week or time of the event).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">Regarding the clinical manifestations, it was found that 1243 teenagers had signs and/or symptoms, with neuro/psychic/muscular manifestations accounting for 35.7% of these, followed by digestive (23.9%) and cardiovascular (15.0%) manifestations. In addition, 206 cases (14.3%) of suicide attempts had depression as their primary cause, followed by family issues (152 cases; 10.3%), problems with relationships (109 cases; 7.4%), anxiety and anger crisis (25 cases; 7.4%) and death of close family member (15 cases; 1.0%), personal problems (12 cases; 0.8%), bad feelings (12 cases; 0.8%), sexual abuse (11 cases; 0.8%) and other motivations (31 cases; 2.2%). It should be mentioned that the motivation could not be determined in 877 cases (60.7%).</p><p id="par0100" class="elsevierStylePara elsevierViewall">The medication use was the most common method (81.1%), followed by drug abuse (4.4%), rodenticides (4.0%), household cleaners (3.5%), pesticides (3.2%), veterinary products (1.4%), residential or industrial chemicals (1.0%), cosmetics and personal hygiene products (0.5%), foods (0.4%), household insecticides (0.3%), plants/fungi (0.1%) and other groups of agents (0.1%). Paracetamol (6.1%), dipyrone (5.0%), fluoxetine (4.7%), clonazepam (4.3%), and amitriptyline (4.0%) were the medications most commonly used in suicide attempts, which are included in the classes of non-opioid analgesics and medications that act on the central nervous system. Additional drugs are listed in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0105" class="elsevierStylePara elsevierViewall">Most cases attended had mild outcome (<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>946; 65.5%). Severe and moderate outcome was developed by 17.0% and 3.9% of cases, respectively. Only three deaths were identified. Thus, the prevalence of moderate, severe or fatal outcome was 21.1%, higher in males (26.5%), with time from exposure to contact to CIATox-Londrina equal to or greater than 300<span class="elsevierStyleHsp" style=""></span>min (27.3%), occurred in 2020 (29.0%), and in January and December (31.0%). When evaluated the variables according to the outcome, in univariate analysis, it was verified that the male adolescents (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.014), cases whose time until attendance by CIATox-Londrina was equal to or greater than 300<span class="elsevierStyleHsp" style=""></span>min (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), events that occurred in 2019 and 2020 (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) and in January and December (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) were associated with moderate, severe or fatal outcome. The other variables it was not associated (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.05) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Logistic regression analysis with a stepwise-backward method was performed (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). In the first model, all variables were analyzed with forced entry and demonstrated that male sex (OR 1.56; 95% CI 1.14–2.14), time to exposure (>300<span class="elsevierStyleHsp" style=""></span>min) (OR 1.70; 95% CI 1.27–2.25), month of exposure (January and December) (OR 1.81; 95% CI 1.31–2.49), and year of occurrence (2019 and 2020) (OR 1.88; 95% CI 1.21–2.99; OR 2.86; 95% CI 1.83–4.58), were associated with moderate, severe and fatal outcome (AIC<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1400.08, BIC<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1463.13). The model 2, after stepwise exclusion of variables without contributions to the model, male sex (OR 1.52; 95% CI 1.11–2.07), time to exposure to contact CIATox (>300<span class="elsevierStyleHsp" style=""></span>min) (OR 1.72; 95% CI 1.29–2.29), month of exposure (January and December) (OR 1.81; 95% CI 1.31–2.48), and independently associated with greater severity of the outcome. In this same model, it was demonstrated that the year of occurrence (2019 and 2020) (OR 1.87; 95% CI 1.21–2.98; OR 2.80; 95% CI 1.31–2.48), the severity of cases was higher, regardless of the data analyzed (final AIC<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1396.74; BIC<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>1433.54). No significant interactions were found between the categorical variables tested. None of the independent variables showed multicollinearity (VIF<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>2).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0115" class="elsevierStylePara elsevierViewall">Adolescence is a period of growing up, which brings new obligations, emotional struggles, and a lot of social and familial demands. These elements – whether or not linked to substance abuse – combined with cognitive immaturity, poor judgment, and age-related impulsivity can cause psychosomatic distress in adolescents, which can lead to suicide conduct.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">3,15,16</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">The analysis of the present study revealed a predominance of suicide attempts in the older age group of adolescents (15–19 years), as well as a higher frequency in females, similar to other national<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">9,17</span></a> and international<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">5,18</span></a> studies. On the other hand, the most serious cases were in males. Even yet, McLoughlin et al. also reported that male adolescents had a greater proportion of moderate, severe or fatal outcomes.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">18</span></a> These higher rates of suicide deaths among males may be associated with a greater propensity to aggravating risk factors for suicide, such as substance abuse, higher levels of aggression (prone to violence), and the use of more aggressive and potentially lethal methods in suicide attempts, including overdose of drugs, psychoactive substances, or poisons.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">19,20</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">As for the substances ingested, drugs were mostly the main means of suicide attempts, a finding that is also shown by Santos et al.,<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">20</span></a> who also attested that, despite being accessible to both genders, women usually use pharmaceuticals more frequently. The availability of drugs in residential settings – self-medication, disregard for medical advice, and overprescription of drugs – enables access and influences events in this way.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">2,15,20</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">The most often utilized drug classes in suicide attempts were non-opioid analgesics, specifically acetaminophen, and dipyrone. This may be because they are widely accessible as they are over-the-counter medications. Then there are the drugs that act on the central nervous system (fluoxetine, clonazepam, amitriptyline, sertraline, carbamazepine, lithium carbonate), a result also found in the study by Lôbo et al.,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">2</span></a> with emphasis on the use of antidepressants.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">3,8,19,21</span></a> Parents who undergo drug treatment for psychiatric disorders, as well as the increase in the number of diagnoses in the adolescent age group, favor easy access to products.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">20</span></a> This is confirmed by the fact that many of the suicide attempts were motivated by depression, usually associated with the use of these drugs.</p><p id="par0135" class="elsevierStylePara elsevierViewall">In our analysis, it was possible to confirm a rise in more significant toxicological incidents among teenagers, particularly in 2019 and 2020 compared to 2017. Additionally, Monday and the night period (06:00–11:59 pm) were the weekdays and day periods with the highest frequency of suicide attempts, respectively. This finding is similar to Zakharov and collaborators’ findings.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">15</span></a> In this sense, a study carried out by Kim et al.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">22</span></a> revealed an increased risk of suicide attempts on Mondays, particularly in adolescents and young people in their 20s, which can be justified due to the intensification of stress and pressure inherent to the beginning of a week.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Furthermore, seasonal variations in the number of adolescent suicide attempts reflect the same pattern described in the literature, with peaks during the spring.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">15,23–25</span></a> These studies, however, only show a seasonal fluctuation in violent suicides that exclude toxicological incidents. In addition to demonstrating a positive correlation between higher ambient temperature (over the past 10 days) and the daily rate of suicide attempts among young men, independent of seasonal effects, Akkaya-Kalayci and collaborators’ analysis revealed that summer was the season when young people aged 15–25 were most likely to attempt suicide, followed by spring.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">26</span></a> These hypotheses are also in line with our results, which show that suicide attempts peak in October and November and decline in the colder months. Nevertheless, suicide attempts that occurred in the summer, despite not representing the majority in absolute numbers, were statistically associated with a more serious outcome. This fact is possibly justified due to the stress related to studies and the end of cycles, the frustration resulting from poor performance in the entrance exams, and the proximity to the end-of-year festivities.</p><p id="par0145" class="elsevierStylePara elsevierViewall">A potential pathophysiological theory holds that exposure to cold nights in the spring and early summer, followed by the body's subsequent daytime heating, hyperactivates the brown adipose tissue, which is abundant in adolescents and contributes to heat intolerance, increased psychomotor anxiety, agitation, and suicidal behavior. Ambient temperature changes also influence serotonin neurotransmission in young people, encouraging impulsivity and suicide attempts.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">23–26</span></a> Finally, due to changes in circadian rhythm and sleep-wake cycle, increased photoperiod in the spring might potentially result in suicidal behavior.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">25</span></a> Further research is essential from a clinical and epidemiological perspective because there are not many studies of seasonality in the adolescent age group in the literature.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Regarding motivation, the study discovered that family issues and depression diagnoses were the two main factors associated with suicide attempts, although the present information should be viewed with caution, as 60% of the cases did not explain the reason for the suicide attempt. Major depressive disorder, dysthymic disorder, and bipolar disorder are the three mood disorders that are frequently mentioned in the literature as being most strongly linked to suicidal conduct.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">7,8,25</span></a> Nock et al. study's<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">27</span></a> found that depressed teenagers are six times more likely to attempt suicide. In addition, it is estimated that in 50% of cases of suicide in young people, family factors are involved, including direct conflicts with parents, absence/negligence of communication, and a history of mental disorders among family members.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">7</span></a> In addition, it is estimated that in 50% of cases of suicide in young people, family factors are involved, including direct conflicts with parents, absence/negligence of communication, and a history of mental disorders among family members.</p><p id="par0155" class="elsevierStylePara elsevierViewall">More serious outcomes (moderate, severe or fatal) were associated with cases where the interval between exposure and treatment by CIATox-Londrina was equal to or more than 300<span class="elsevierStyleHsp" style=""></span>min Possible contributing factors are the different time intervals from ingestion to initiation of medical treatment; the strict guidelines for carrying out decontamination measures, as well as the limited reliability of dose estimates based primarily on indirect data.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">15</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">Concerning the gathering, analyzing, and interpreting of data, the study had a number of limitations. Most teenagers do not seek out medical attention, and contact with CIATox-Londrina is optional this regard, the recorded number of non-fatal and primarily less serious suicide attempts does not accurately reflect the actual number, with incidents that go unreported. Still, despite losses due to lack of final outcome data (<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>17) and information on ethnicity (<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>4) and time from exposure to contact with CIATox (<span class="elsevierStyleItalic">N</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>28), this total is equivalent to only 3.0% of the total number of cases. In addition, some uncertainties cannot be avoided, such as which substances were ingested and at what doses, so the estimates end up being based mainly on indirect data and with limited reliability.</p><p id="par0165" class="elsevierStylePara elsevierViewall">In conclusion, suicide attempts due to toxicological events in adolescents have a higher prevalence of negative outcomes (moderate, severe or fatal) in males and during the summer months, with evidence of an increase in the severity of cases over the years. The scenario of the COVID-19 pandemic, concomitant with isolation, uncertainty, and fear of losing loved ones, can make adolescents even more vulnerable, causing or aggravating suffering and, consequently, increasing the risk of suicidal behavior. Since adolescents need a more rigorous psychosocial follow-up due to the adversities inherent to the age group, there is a clear need for better identification of striking prodromal signs of suicide attempts in order to develop therapeutic interventions and preventive strategies.
